Objective Identify clinical-functional factors associated to the risk of falls, assessed by Mini-BESTest in older adults with type 2 diabetes mellitus (T2DM). Method This cross-sectional study. A total of 145 older adults aged ≥60 years were evaluated through sociodemographic variables (sex, age group, married, education level, general health status hearing and vision), Mini-BESTest, Mini-Mental State Examination (MMSE), Geriatric Depression Scale (GDS-15) and dual-task Timed Up and Go Test (TUG) Multiple logistic regression model was used. Results The sensory orientation domain presented the highest average score, followed by the gait stability, anticipatory postural adjustments and postural responses domains. Factors associated to the risk of falls in older adults are: poor/very poor visual perception OR 3.40 (1.50-7.72); have respiratory diseases OR 8.00 (1.32-48.46); feeling dizzy OR 2.53 (1.10-5.80); and TUGT (dual task) time equal to or greater than 13.5 seconds OR 3.31 (1.03-10.64). Conclusion Older adults in this study presented impaired postural balance, mainly in the postural responses domain. The knowledge of the factors associated with the risk of falls in older adults with T2DM allows for better guidance in prevention, assessment and intervention, in order to minimize the occurrence of falls and maintain or optimize postural balance. Several factors influenced this outcome, such as overweight, low physical activity and education, several comorbidities, polypharmacy, T2DM diagnosis for more than ten years, negative perception of general health and vision, and depressive symptoms.

O presente estudo teve como objetivo investigar o equilíbrio postural das pessoas idosas com e sem Diabetes Mellitus tipo 2 (DM2) e verificar as características clínicas associadas. Trata-se de um estudo observacional, analítico e transversal; avaliou-se 233 pessoas idosas (60 anos ou mais), sendo 147 com diagnóstico de DM2 (G1) e 86 sem DM2 (G2). O equilíbrio postural foi avaliado por meio do Mini - The Balance Evaluation Systems Test (MiniBESTest). As associações entre os escores totais do G1 e G2 foram verificadas por meio dos testes de Mann-Whitney e teste Qui-Quadrado, α=0,05. Após a análise inferencial, foi observada significância entre as variáveis dos grupos G1 e G2: IMC (p=0,048), número de medicamentos (p=0,001), TUGt motora (p=0,015), FPP (p<0,001), WHODAS (p=0,011), EDG (p=0,001), presença de alterações nos pés (p<0,001), calosidade (p<0,001), alterações articulares (p<0,001), alterações dermatológicas (p<0,001), sensibilidade vibratória (p<0,001), sensibilidade cutâneo-protetora (p<0,001), hipotensão ortostática (p=0,003) e medo de quedas (p=0,002). A média do MiniBesTest foi de 22,15±4,64 pontos para o grupo com DM2 e 21,94 ± 4,89 pontos para o grupo sem DM2. Foi observado também, significância nos domínios do Mini BESTest: ajustes antecipatórios (p=0,001) e respostas posturais (p=0,042) em idosos com e sem DM2. Os idosos do G1 apresentam maior prejuízo do equilíbrio postural, quando associados à presença de sintomas depressivos, diminuição da mobilidade, presença de comorbidades, polifarmácia, alterações nos pés e medo de quedas.

Adherence to Medication in Older Adults with Type 2 Diabetes Living in Lubuskie Voivodeship in Poland: Association with Frailty Syndrome

Purpose: Diabetic patients aged 65 years or older are more likely to be frail than non-diabetic older adults. Adherence to therapeutic recommendations in the elderly suffering from diabetes and co-existent frailty syndrome may prevent complications such as micro- or macroangiopathy, as well as significantly affect prevention and reversibility of frailty. The study aimed at assessing the impact of frailty syndrome (FS) on the level of adherence to medication in elderly patients with type 2 diabetes (DM2). Patients and Methods: The research was carried out among 175 DM2 patients (87; 49.71% women and 88; 50.29% men) whose average age amounted to 70.25 ± 6.7. Standardized research instruments included Tilburg frailty indicator (TFI) to assess FS and adherence in chronic disease scale questionnaire (ACDS) to measure adherence to medications. Results: The group of 101 (57.71%) patients displayed medium, 39 (22.29%)—low, and 35 (20.00%)—high adherence. As many as 140 of them (80.00%) were diagnosed with frailty syndrome. The median of the average result of TFI was significantly higher in the low adherence group (p ˂ 0.001) (Mdn = 9, Q1–Q3; 7–10 pt.) than in the medium (Mdn = 6, Q1–Q3; 5–9 pt.) or high adherence (Mdn = 6.00, Q1–Q3; 4.5–8 pt.) ones. The independent predictors of the chance to be qualified to the non-adherence group included three indicators: TFI (OR 1.558, 95% CI 1.245–1.95), male gender (OR 2.954, 95% CI 1.044–8.353), and the number of all medications taken daily (each extra pill decreased the chance of being qualified to the non-adherence group by 15.3% (95% CI 0.728–0.954). Conclusion: Frailty syndrome in elderly DM2 patients influenced medical adherence in this group. The low adhesion group had higher overall TFI scores and separately higher scores in the physical and psychological domains compared to the medium and high adhesion groups.
