Objective: To calculate the percentage of women treated at the Climaterium Outpatient Clinic of Hospital Electro Bonini who had a high risk of fracture calculated by FRAX to offer empirical treatment for osteoporosis. Methods: A FRAX ™ questionnaire was applied during consultations at the Climacteric Ambulatory in September 2018. Women under 40 and over 90 years of age and cancer from anywhere were excluded. Results: Of the 74 questionnaires answered, 24 (32.43%) had an increased risk of fracture, and patients were considered to start treatment for osteoporosis. Conclusion: Despite its limitations, the FRAX™ tool is an easy method to apply and estimates the risk of bone fracture on an outpatient basis, helping with early therapeutic management, especially when there is a lack of access to bone densitometry.

Background: Identifying postmenopausal women with a high risk of having osteoporosis and fractures is a current challenge. This study aimed to assess the diagnostic performance of biochemical tests in identifying secondary osteoporosis and the fracture risk assessment tool (FRAX) in identifying fracture risk.Methods: Data from biochemical tests and bone densitometry of postmenopausal women were analyzed. Additionally, the FRAX result was obtained and the patients were classified according to the National Osteoporosis Guideline Group (NOGG).Results: A total of 646 women were evaluated, of whom 201 (31.1%) had osteoporosis or a previous frailty fracture. These women had statistically different parathyroid hormone (PTH) and alkaline phosphatase serum levels (p<0.01 and p=0.02, respectively) than those without osteoporosis or fracture. However, those at high risk had a higher prevalence of hypovitaminosis D (46% vs. 36%) and hypocalciuria (17% vs. 9%). The FRAX showed an area under the curve of 0.757 (p<0.01) and 0.788 (p<0.01) for identifying women at risk for “major fractures” and “hip,” respectively. The NOGG categorization had a sensitivity of 19% to identify high-risk women, a specificity of 91.3% for low-risk women, with a positive predictive value of 57.4% and a negative predictive value of 64.6%.Conclusions: The evaluation of PTH, 25-hydroxy-vitamin D, serum calcium, and 24-hr urinary calcium proved adequate for initial osteoporosis screening. The FRAX tool has a regular ability to screen women at risk for fracture, and the NOGG method has high specificity to identify those at low risk.

A osteoporose é caracterizada pela fragilidade óssea e mudanças em sua microarquitetura. O seu principal desfecho clínico é a fratura por baixo impacto. Uma das principais ferramentas de avaliação clínica do risco de fratura osteoporótica é o FRAX. Este trabalho objetiva avaliar os fatores de risco para osteoporose e a indicação de tratamento em idosos. Foi realizado um estudo transversal, descritivo, com abordagem quantitativa e qualitativa, por meio de um questionário, com 145 pacientes que foram atendidos no setor ambulatorial do DeCos Day Hospital, no período de novembro de 2017 a abril de 2018. Observou-se que a osteoporose prevaleceu no sexo feminino, sedentários e com idade média de 71 anos. A presença de fratura predominou na faixa etária de 70 a 79 anos e nenhum participante fazia tratamento específico para a osteoporose. Conclui-se que orientações dietéticas saudáveis são importantes como medida preventiva e a ferramenta FRAX poderia ser utilizada para estimar a probabilidade de fratura, visto que a densitometria não é a acessível a toda a população.
