ObjectiveThe aim of this study was to identify patterns related to health and their association with chronic kidney disease (CKD) in the Brazilian population.MethodsWe used data from the National Health Survey (PNS), 2019. Participants were interviewed and answered questions related to socioeconomic and demographic information (gender, age, education, race/color), health conditions (presence of hypertension, diabetes mellitus, hyperlipidemia, cardiovascular disease, overweight and CKD) and lifestyle (smoking, alcohol consumption, physical activity and food consumption). To identify patterns, we used exploratory factor analysis. We performed logistic regression models to describe the association of CKD with each pattern in crude models and adjusted for gender, age group, education level and race/color.ResultsA total of 90,846 individuals were evaluated. The prevalence of CKD was 1.49% (95% CI: 1.3–1.6). Three health-related patterns – metabolic factors, behavioral risk factors and behavioral protective factors – were identified by factor analysis. Metabolic factors were determined by the presence of hypertension, diabetes mellitus, hyperlipidemia and cardiovascular diseases. Behavioral risk factors were determined by smoking, alcohol consumption, regular consumption of soft drinks, sweets and artificial juices, and high salt consumption. The protective behavioral factors were established by the practice of physical activity and regular consumption of vegetables and fruits. Participants of the highest tertile for metabolic factors were more likely to have CKD in the adjusted model (OR = 3.61, 95% CI: 2.69–4.85), when compared to those of the lower tertile.ConclusionThe pattern referring to metabolic factors was associated with a higher chance of presenting CKD.

Introdução: A alteração na força de preensão (FP) manual é indicador para surgimento de algumas doenças crônicas, como a diabetes mellitus (DM) e hipertensão arterial sistêmica (HAS). Objetivo: Analisar a relação da FP com a composição corporal e indicadores laboratoriais de diabéticos e hipertensos assistidos na Atenção Primária à Saúde. Métodos: A amostra foi composta por 185 usuários de uma Unidade Básica de Saúde na cidade de Santarém,Pará, sendo distribuídos em dois grupos: controle (CTL) – usuários sem o diagnóstico para DM e/ou HAS (n=66); e DM/HAS (n=119) – usuários com DM ou HAS ou as duas doenças. A coleta de dados envolveu informações sociodemográficas, clínicas, antropométricas, bioquímicas e FP. Os dados foram analisados por estatística descritiva e inferencial, adotando-se p<0,05. Resultados: Notou-se que a FP baixa no grupo DM/HAS apresentou associaçãocom valores elevados do índice de massa corporal, circunferência abdominal, percentual de gordura, massa gorda, colesterol total, triglicerídeos e com mais fatores para a síndrome metabólica (p<0,05). Já o grupo CTL, em relação do DM/HAS para uma mesma classificação de FP, demonstrou significância para menores valores pressóricos, do índice de massa corporal, da circunferência abdominal, percentual de gordura, massa gorda, bem como menor chance para o desenvolvimento da síndrome metabólica (p<0,05). Conclusão: Conclui-se, conforme a proposta do estudo, que é relevante a avaliação e acompanhamento da FP em indivíduos com doenças crônicas, em especial a DM e a HAS, a fim de monitorar a adiposidade corporal e a dislipidemia, evitando o agravo das doenças instaladas ou o surgimento de novas.

INTRODUCTIONChronic kidney disease (CKD) is a significant global public health problem with a major socioeconomic impact. 1,2 Its worldwide prevalence is estimated at 10-16%; 1,3,4 additionally, its prevalence has increased in the recent decades, along with the number of patients with terminal CKD requiring transplantation, 3,6 mostly due to the increase in the prevalence of hypertension and diabetes mellitus. 4,5 Patients with CKD are prone to skin abnormalities. 7,8 These manifestations are often associated with impaired renal function and are more prevalent in end-stage disease, 7,8 when the kidneys are unable to maintain appropriate levels of metabolic products, such as urea, creatinine, sodium, calcium, and phosphate, causing damage to several organs, including the skin. 9 Kidney transplantation is the best treatment for patients with end-stage CKD; 3,10 however, the immunosuppression required to maintain the graft can lead to various side effects and a greater susceptibility to infectious and neoplastic diseases. 11 Besides immunosuppression itself, the mechanisms of action of immunosuppressive drugs and viral infections (oncogenic viruses) are associated with cutaneous disorders in renal transplant recipients (RTR). 12,13
