Educative Process With Relatives of Children With Special Health Needs in the Hospital-Home Transition

“…Regarding the admission of CSHCN, it was evident that in most SADs in SC it is carried out by workers from EMAD, with emphasis on the nurse who is usually responsible for the educational process with the families and for identifying the needs, strengths, and weaknesses in the execution of care.
