The More Doctors Program has been considered, in recent studies, the most important normative change of the State’s attributions in the organization of the education of human resources for the Brazilian National Health System (SUS). This article analyzes the most structuring axis of the program, education in health, by revising literature, and analyzing documents and databases in order to describe its model as a public policy and discuss which normative changes had an effect on the State’s action. The article identifies institutional changes in the Brazilian Ministry of Health and Ministry of Education, in the reorientation of medical education, in the expansion and redistribution of undergraduate and residency seats, and in the creation of public policy instruments to plan, regulate and assess the education of specialists. The paper is concluded by identifying interruptions in the program’s structuring actions and objectives, and goals and objectives that will probably not be met within the established deadlines.

Réplica Reply RespuestaAs observações dos debatedores se somam ao esforço empreendido por este grupo de autores implicados com a crítica e aprimoramento da política em análise, esforço do qual faz parte tanto o seminário internacional que motivou o artigo quanto o próprio.Em linhas gerais, todos os debatedores reconheceram as questões sociais que justificaram a criação do Programa Mais Médicos (PMM), notadamente a falta e a má distribuição de médicos e a inadequada formação desses profissionais (qualitativamente e quantitativamente) para as necessidades do Sistema Único de Saúde (SUS). Entendem o PMM como importante para enfrentar estas questões e analisam efeitos positivos do mesmo. Concordando com a maioria das observações, dado o espaço da réplica, focaremos nas observações mais críticas.Em primeiro lugar, é necessário compreender contextualmente e globalmente o PMM. Ele é parte de um momento de priorização da Atenção Básica (AB) que, nos últimos quatro anos, teve aumento expressivo do financiamento (o orçamento federal ampliou-se em mais de 100%¹), inédito investimento de 5,6 bilhões em estrutura e ampliação e diversificação das modalidades de equipes multiprofissionais; contudo, a falta e má distribuição de médicos condicionou a expansão do acesso com qualidade a um contingente importante da população².De outro lado, é necessário perceber o PMM como um conjunto articulado de ações de curto, médio e longo prazo. O Provimento Emergencial já garantiu a cobertura de cinquenta milhões de brasileiros e chegará a 63 milhões ainda em 2015³. Diferente de 2013 e 2014, quando os médicos com registro no Brasil ocuparam aproximadamente 10% das vagas, em 2015 ocuparam 92% das novas vagas, reduzindo consideravelmente a necessidade de médicos estrangeiros para os próximos anos. As principais dificuldades apontadas pelo Tribunal de Contas da União, decorrentes de avaliação realizada quando da implantação do programa 4 já foram atualmente superadas 5 , com destaque para: a ampliação e qualificação da supervisão, que recebeu nota 9,3 (em escala de zero a dez) dos médicos brasileiros 3 ; e a constatação do importante aumento do número de médicos, equipes, cobertura e consultas na AB 5 . Outro destaque é que a Lei 12.871 6 não prevê data para o fim do programa, e sim determina que a autorização concedida a cada profissional para o exercício da medicina sem registro no Brasil seja de três anos (prorrogável por mais três), tempo maior que o tempo médio de permanência dos médicos nas equipes de saúde da família no período anterior ao programa 7 . Esclarecido isso, é possível que, a exemplo dos EUA e Austrália, o PMM possa seguir por anos como estratégia de provimento, a cada ano com mais brasileiros e mais dirigido às áreas que, mesmo com as demais ações do programa, sigam com dificuldade de atrair médicos. A meta fixada pelo programa deve atingir 2,7 médicos/mil habitantes em 2026, por meio da expansão de vagas de graduação em medicina, em instituições públicas e privadas, de modo que o país saia da insuficiente oferta de 0,8 vag...

The education and provision of health professionals are the main strategies to increase capacity and respond to health systems' needs among countries worldwide. Recently, the Brazilian government passed a law to create a national program, called the More Doctors Program, to improve the capacity to respond to the demand for doctors in underserved areas. The law was designed with three main axes: provision, education and infrastructure. The first, provision, would increase the provision of medical doctors through monetary and non-monetary incentives to attract national and foreign doctors to work in remote areas. The education axis was related to opening new Courses and Institutions to graduate new doctors in remote areas. The third axis was to improve primary care facilities. However, the most controversial aspect of this Program was the partnership between Cuba and Brazil, through an international cooperation mediated by the Pan-American Health Organization. It involved 18,240 new primary care physicians. Focusing on provision, mainly in the Brazil-Cuba international cooperation, a qualitative study was designed and conducted, analyzing the More Doctors policy cycle, using Howlet & Ramesh, 2003 as well as Ball, 1992 as a reference. This study examined the five stages of the policy cycle: agenda preparation, policy formulation, decision making, implementation and evaluation and Practitioners. In addition, there could be an influence of the Cuban socialist model. The Cuban doctors bring a new perspective to Brazilian health professionals on how to build linkages with the users and how to deal with poverty and inequity. On the micro level, both groups benefited from ongoing learning strategies, supervisions in locus, distance learning courses, round tables on the main health problems, group practice sharing, and the interchange on health local planning. The main problems involve the temporary nature of the provision strategy, which is not well resolved with the Brazilian medical corporations and professional bodies. Furthermore, the supervisions are not always well organized by the Universities. The study also shows the fragilities of federative integration regarding policy implementation and management.
