Aims: The purpose of this study was to determine the impact of strain elastography (SE) on the Breast Imaging Reporting Data System (BI-RADS) classification depending on invasive lobular carcinoma (ILC) lesion size.Materials and methods: We performed a retrospective analysis on a sample of 152 female subjects examined between January 2010 – January 2017. SE was performed on all patients and ILC was subsequently diagnosed by surgical or ultrasound-guided biopsy. BI-RADS 1, 2, 6 and Tsukuba BGR cases were omitted. BI-RADS scores were recorded before and after the use of SE. The differences between scores were compared to the ILC tumor size using nonparametric tests and logistic binary regression. We controlled for age, focality, clinical assessment, heredo-collateral antecedents, B-mode and Doppler ultrasound examination. An ROC curve was used to identify the optimal cut-off point for size in relationship to BI-RADS classificationdifference using Youden’s index.Results: The histological subtypes of ILC lesions (n=180) included in the sample were luminal A (70%, n=126), luminal B (27.78%, n=50), triple negative (1.67%, n=3) and HER2+ (0.56%, n=1). The BI-RADS classification was higher when SE was performed (Z=- 6.629, p<0.000). The ROC curve identified a cut-off point of 13 mm for size in relationship to BI-RADS classification difference (J=0.670, p<0.000). Small ILC tumors were 17.92% more likely to influence BI-RADS classification (p<0.000).Conclusions: SE offers enhanced BI-RADS classification in small ILC tumors (<13 mm). Sonoelastography brings added value to B-mode breast ultrasound as an adjacent to mammography in breast cancer screening.

Devido a subjetividade na interpretação de imagens, os sistemas de diagnóstico auxiliado por computador (CADx) têm oferecido ao especialista uma segunda opinião mais precisa e confiável. Nesse propósito, essa pesquisa apresenta uma metodologia de investigação da potencialidade diagnóstica de um sistema computacional na classificação de achados suspeitos em imagens de ultrassom modo-B e de elastografia da mama. A base de dados foi constituída por 31 lesões malignas e 52 benignas e um conjunto adicional contendo 206 lesões de ultrassom modo-B (144 benignas e 62 malignas) para a realização dos testes de aprendizado de máquina. O contorno foi determinado automaticamente e através do delineamento manual de três radiologistas sob a imagem de ultrassom modo-B e, em seguida, mapeado na imagem elastográfica. As lesões foram classificadas pelo sistema CADx desenvolvido para ultrassom modo-B e elastografia do tipo strain. Os dados foram avaliados por meio da sensibilidade, especificidade e AUC. O sistema CADx desenvolvido proporcionou equivalência diagnóstica para a classificação das lesões a partir das diversas formas de determinação do contorno (manual e automática), permitindo a redução da variabilidade. Objective We herein compared the diagnostic accuracy of the BI‐RADS, ABVS, SWE, and combined techniques for the classification of breast lesions. Methods Breast lesions were appraised using the BI‐RADS classification system as well as the combinations of BI‐RADS plus ABVS (BI‐RADS + ABVS) and BI‐RADS plus SWE (BI‐RADS + SWE), and both methods (BI‐RADS + ABVS + SWE) by two specialties Medical Ultrasound physician. The Fisher's exact and χ2 tests were performed to compare the degree of malignancy for the various methods with a pathology ground truth. Receiver operating characteristic curves (ROC) were generated and the corresponding area under the curve (AUC) values were determined to test the diagnostic efficacy of the various methods and identify the optimal SWE cut‐off indicative of malignancy. Results The incidence of the retraction phenomenon on ABVS images of the malignant group was significantly higher (P < .001) than that of the benign group. The specificity, sensitivity, and positive and negative predictive values of the BI‐RADS classification were 88.72, 79.38, 83.70, and 85.50%, respectively. BI‐RADS plus SWE‐Max exhibited enhanced specificity, sensitivity, and positive and negative predictive values of 88.72, 92.78, 85.70, and 94.40%, respectively. Similarly, when BI‐RADS + ABVS was utilized, the sensitivity and negative predictive value increased to 95.88 and 96.40%, respectively. BI‐RADS + ABVS + SWE possessed the highest overall sensitivity (96.91%), specificity (94.74%), and positive (93.10%) and negative (97.70%) predictive values from all four indices. Conclusion ABVS and SWE can reduce the subjectivity of BI‐RADS. As a result, BI‐RADS + ABVS + SWE resulted in the best diagnostic accuracy.
