Introduction. The goal of this systematic review and meta-analysis was to consolidate the available data on the role of the neutrophil to lymphocyte ratio (NLR) in predicting adnexal torsion (AT), to help guide clinical decision-making and outcomes. Methods. We used Web of Science, PubMed, and Scopus to conduct a systematic search for relevant publications published before June 26, 2022. We reported standardized mean difference (SMD) with a 95% confidence interval (CI). Because a significant level of heterogeneity was found, we used the random-effects model to calculate pooled effects. We used the Quality Assessment of Diagnostic Accuracy Studies 2 (QUADAS-2) too for quality assessment. Results. Overall, 15 articles were included in the analysis. A random-effects model revealed that patients with AT had elevated levels of NLR compared to those with other adnexal masses ( SMD = 1.06 , 95 % CI = 0.67 to 1.45, p < 0.001 ). So, NLR had diagnostic value. In the subgroup analysis according to ethnicity, we found that Caucasian patients with AT had elevated levels of NLR compared to patients who were operated due to adnexal mass and reported as having a benign ovarian cyst, without torsion ( SMD = 1.12 , 95 % CI = 0.71 to 1.54, p < 0.001 ). However, in the case of East Asian patients, there was no difference between cases and controls ( SMD = 0.86 , 95 % CI = − 0.21 to 1.94, p = 0.11 ). The pooled sensitivity of NLR was 0.79 ( 95 % CI = 0.72 – 0.85 ), and the pooled specificity was 0.84 (95% CI, 0.74–0.91). Conclusion. In conclusion, there has been an interest in the use of NLR as a diagnostic marker for AT.

Aim: Ovarian torsion is among the gynecological emergencies. Laboratory and imaging methods are used for diagnosis. Symptoms are generally nonspecific, making it difficult to differentiate from other acute abdominal emergencies. This study aims to investigate the diagnostic value of neutrophil-lymphocyte ratio (NLR) in addition to imaging methods for the diagnosis of ovarian torsion. Material and Methods: A total of 120 patients were included in the study. The study was planned retrospectively. The patients included in the study were analyzed in three different groups. Those with follicular cysts were in Group 1, those with simple cysts were in Group 2, and those with ovarian torsion were in Group 3. The groups were compared in terms of laboratory parameters and demographic characteristics. Adnexal Torsion in Adolescents – a Retrospective Analysis from a Tertiary Centre in Latvia

Objective - To review the data of cases of adnexal torsion in the paediatric population in a national level centre in Latvia and examine the pitfalls of diagnostic and therapeutic strategies.Methods - A retrospective review of medical records was performed on 61 cases of suspected and/or confirmed adnexal torsion in a national level paediatric centre.Results - In 85.5% of cases the initial diagnosis was made correctly. The age of the patient was a significant contributor to a shorter time until surgery (P=0.016). Abdominal pain was the most common symptom (90.6%), followed by nausea (58.5%) and vomiting (52.8%). Only 62.3% of the torsion patients had an elevated WBC and 69.4% had no elevated body temperature. The shorter the duration of symptoms, the more likely it was that the organ salvage procedure was performed (P=0.021). The sensitivity of ultrasound for the correct diagnosis was 71.4%. In almost a third of the torsion cases, no pelvic lesion was detected on ultrasound (26.5%). When ultrasound suspected torsion, the time before operation was significantly shorter (P<0.001), the presence of blood flow in the ovary extended the time until surgery (P=0.026). Organ-sparing surgery was performed in 73.6% of the cases.Conclusion - Medical professionals should not exclude the diagnosis of adnexal torsion in the absence of fever or leucocytosis, as well as in the case of normal adnexa on ultrasound. A delay in surgery increases the risk of irreversible ischemic damage leading to organ loss.
