Background Thoracentesis yields valuable insights in pleural effusion diagnosis by accurately interpreting pleural fluid analysis results and can cause several complications, which emphasizes the importance of training in a simulated environment. There are many expensive simulators related to this procedure and few validated scenarios. This study aimed to build and validate a low-cost simulator and a clinical simulation scenario for teaching the thoracentesis surgical technique to undergraduate medical students. Methods This is a methodological study carried out at a public university in the interior of the state of São Paulo, Brazil. It was carried out in three methodological stages, namely: 1) Construction of the simulator, which involved planning, surveying, pricing, and use of material resources 2) Construction of a simulated thoracentesis scenario, based on literature and a simulation script and 3) Validation by experts and pilot study of the simulator and scenario. Experts were selected according to Fehring criteria. Results The simulator proved to be suitable and low-cost (US $18). Modifications to the scenario were suggested by the experts and students in the pilot study, with 100.0% agreement. Conclusions In conclusion, a low-tech, handmade, and low-cost simulator was built and validated for training in the thoracentesis surgical technique, as well as a clinical simulation scenario for the management of patients with pleural effusion, which can be included in various medical teaching contexts.

Educação na Saúde (SGTES) do Ministério da Saúde do Brasil formalizou um intercâmbio de políticas e práticas sobre demografia médica, avaliação de competências dos profissionais da saúde e centros de simulação realística com a Conselheria de Igualdade, Saúde e Políticas Sociais da Junta de Andaluzia, Espanha. Após as mudanças decorrentes da publicação da Lei 12.871/2013 (Programa Mais Médicos para o Brasil), colocou-se em pauta a ordenação da formação de profissionais de saúde pelo SUS, postulando-se nessa parceria, entre outras ações, a transferência de tecnologia para construção de centros de simulação realística em saúde no Brasil. A partir deste primeiro acordo e de oficinas de formação e desenvolvimento, a Ebserh foi incorporada à proposta e realizou-se com os professores de Andaluzia uma oficina na Universidade Federal de São Carlos, UFSCar, na perspectiva de mobilizar centros de formação em simulação realística no SUS, voltados para educação permanente de profissionais de saúde e também para formação de estudantes e residentes. Este foi o primeiro movimento que, após anos, resultou na presente carta-acordo que possibilitou à ABEM, em parceria com a OPAS, SGTES, Ebserh, realizar o Projeto "Desenvolvimento da Rede de centros de Simulação Clínica: elaboração de curso para capacitação de multiplicadores".

The aim of this study is to present and analyze the vaginal delivery checklist as a safe and useful learning tool for first-year residents in the gynecology and obstetrics and midwifery specialties at different hospitals and to analyze the items that comprise it and the progress of the residents during the first 30 normal deliveries attended in a supervised manner. We present a descriptive, observational and prospective study in which 18 participants from different hospitals in Spain completed a checklist of skills in the first 30 births attended autonomously after learning its use in a simulation workshop. We collected a total of 329 of 330 checklists completed by 11 participants. In addition, the mean responses for each item contained on the checklist and the mean global progression of the participants were subsequently analyzed, detecting statistically significant differences using Student’s t-test for paired data. During the data collection period between September 2020 and August 2021, a total of 329 checklists were analyzed. The global average of positive responses for the 30 questions contained on the checklist was 25.36 out of 30 points. The fulfillment of the different items was variable, with the lowest fulfillment of 2.18 points, although 73% of the items obtained a score greater than 25. A statistically significant difference in the mean compliance of the first five childbirths compared to the last five deliveries using the proposed checklist was obtained, with p < 0.01. In conclusion, knowing the importance of using a checklist to anticipate risk situations and reduce the number of adverse perinatal outcomes, we can say that, for first-year residents in gynecology and obstetrics attending a clinical simulation workshop in eutocic vaginal delivery, it may be interesting to have a tool, such as the checklist proposed in this study, that facilitates the learning process and the suitable progress of the participants.
