Introduction: Malnutrition is a complex, multi-factorial and common problem in patients with end-stage renal disease (ESRD) undergoing hemodialysis that increases morbidity and mortality rate in them. Objectives: To assess the prevalence of malnutrition and associated factors in hemodialysis patients. Patients and Methods: This cross-sectional study was conducted on 239 hemodialysis patients (162 males and 77 females) referred to three dialysis centers in Ahvaz, Iran in 2018. The nutritional statuswas measured using subjective global assessment (SGA)tool. The anthropometric indices, body mass index (BMI) and biochemical parameters including albumin, triglyceride, C-reactive protein (CRP), and adequacy of dialysis (Kt/V) were also measured in all patients. Results: Based on SGA, 45 (18.8%) patients including 32 males and 13 females had mild to moderate malnutrition while 26 (10.9%) patients consisting of 11 men and 15 women had severe malnutrition. We found a significant association between patients’ gender and malnutrition status (P=0.013). In addition, a significant association was seen between age (P=0.024), BMI (P=0.0001), CRP (P=0.047) and serum albumin concentration (P=0.007) with nutritional status. However, we did not find any significant association between triglyceride, Kt/V and duration of hemodialysis per week with malnutrition (P>0.05). Moreover, BMI, patient’s gender and serum albumin were the significant predictors of severe malnutrition in hemodialysis patients (P<0.05). Conclusion: According to high prevalence of malnutrition in ESRD patients undergoing hemodialysis, periodic assessment of nutritional status is necessary for them. Moreover, female patients, CRP, serum albumin and BMI showed a significant correlation with SGA score, which are important to determine the nutritional status of hemodialysis patients (HD) patients.

Introdução: Na Doença Renal Crônica Avançada, o paciente necessita de uma terapia renal substitutiva, dentre elas, a hemodiálise (HD). Apesar dos benefícios da HD, este procedimento afeta o estado nutricional e a qualidade de vida (QV) do doente. Objetivo: Correlacionar o estado nutricional ao nível de QV de pacientes em HD. Metodologia: Estudo transversal, com pacientes em HD há pelo menos 3 meses. O estado nutricional foi avaliado pelo índice de massa corporal (IMC), circunferência do braço (CB), circunferência muscular do braço (CMB), prega cutânea tricipital (PCT), albumina (Alb) e creatinina (Cr) séricas e a QV pelo Medical Outcomes Study-36 – Item Short Form Health Survey (SF-36). As pontuações geradas pelo SF-36 foram correlacionadas com os indicadores do estado nutricional através do Coeficiente de Correlação de Pearson e/ou Spearman´s. Resultados e Discussão: Participaram do estudo 37 pacientes, com idade média de 50,6 ± 13,0 anos, sendo 54,1% do sexo feminino. Segundo a avaliação do estado nutricional, houve prevalência de eutrofia (48,7%) pelo IMC e desnutrição de 37,8% evidenciada pela CB e CMB. Na avaliação bioquímica, 25,0% estavam com hipoalbuminemia e 18,9% com redução da Cr. A dimensão de QV mais comprometida foi a limitação por aspectos físicos. Na análise das correlações, parâmetros nutricionais (IMC, CB e Alb) estavam ligados à QV dos pacientes. Conclusão: IMC, CB e Alb correlacionaram-se com dimensões do SF-36, indicando que alterações no estado nutricional podem influenciar o funcionamento físico e emocional desses pacientes.

Purpose: Intake of energy and some nutrients is frequently inadequate in dialysis patients because of eating and appetite disorders due to uremic syndrome. Inadequate energy and protein intake cause to malnutrition. Anthropometric measurements, biochemical markers and dietary intake records have great importance in determining the nutritional status of hemodialysis (HD) patients. Therefore this study was planned to determination of daily energy, nutrient intake and some biochemical markers of adult hemodialysis patients. Method and Material: A hundred and thirty six (male = 73, female = 63) clinically stable HD patients were enrolled in this study. All patients were taken 24-hour dietary recall. The daily energy and nutrient intakes of patients were calculated by food consumption records and anthropometric measurements were taken. Results: The mean age was found 45.5±13.51 years. The ratio of patients with body mass index (BMI) below 18.5 kg/m 2 was 7.4% and 25 kg/m 2 or upper was 16.9%. Blood hemoglobin, hematocrit, albumin and total protein levels were below the normal levels. The daily dietary energy and protein intakes were found under the recommended level in the study period. A significant positive correlation was found between daily protein intake and serum albumin levels (r=0.210, p=0.014), daily protein intake and serum total protein levels (r=0.201, p=0.019), daily energy intake and serum total protein levels (r=0.178, p=0.039). Conclusions: HD patients often have low protein and energy intakes and most of them suffer from malnutrition. Therefore nutritional status of patients should be evaluated and periodically nutrition education should be given for improving eating habits and increasing dietary compliance is recommended.
