BackgroundChild maltreatment can have serious effects on development and physical, social and emotional wellbeing. Any long-lasting relational effects can impede the capacity to nurture children, potentially leading to ‘intergenerational trauma’. Conversely, the transition to parenthood during pregnancy, birth and the early postpartum period offers a unique life-course opportunity for healing. This systematic review aims to understand the pregnancy, birth and early postpartum experiences of parents who reported maltreatment in their own childhood.MethodsA protocol, based on the ENTREQ statement, was registered with PROSPERO. We searched Medline, PsycINFO, CINAHL, EMBASE, NHS Evidence and key Web of Science databases from date of inception to June 2018 to identify qualitative studies exploring perinatal experiences of parents who were maltreated in their own childhood. Two reviewers independently screened articles for inclusion and extracted data. Data were synthesised using grounded theory and thematic analysis approaches.FindingsThe search yielded 18329 articles, 568 full text articles were reviewed, and 50 studies (60 articles) met inclusion criteria for this review. Due to the large number of studies across the whole perinatal period (pregnancy to two years postpartum), this paper reports findings for experiences during pregnancy, birth and early postpartum (27 studies). Parents described positive experiences and strategies to help them achieve their hopes and dreams of providing safe, loving and nurturing care for their children. However, many parents experienced serious challenges. Seven core analytic themes encapsulated these diverse and dynamic experiences: New beginnings; Changing roles and identities; Feeling connected; Compassionate care; Empowerment; Creating safety; and Reweaving a future.ConclusionsPregnancy birth and the early postpartum period is a unique life-course healing opportunity for parents with a history of maltreatment. Understanding parent’s experiences and views of perinatal care and early parenting is critical for informing the development of acceptable and effective support strategies.

Introdução: O aborto é o produto da concepção eliminado no abortamento. A decisão em abortar evidencia a não realização de planejamento reprodutivo, a violência doméstica, o abandono paterno e a escassez e/ou oferta de meios econômicos. Objetivo: Analisar como os homens participam do processo de abortamento induzido. Metodologia: Estudo de campo com abordagem qualitativa exploratória, realizada na zona rural do município de Antônio Gonçalves, Bahia. Por meio da técnica snowball, cinco pessoas foram entrevistadas e as narrativas foram processadas pelo Software IRAMUTEQ que gerou a Análise de Similitude. Resultados: Expressa-se a participação ativa e a influência decisiva dos homens no processo de abortamento induzido, por meio da oferta de método abortivo, da negação da paternidade e do abandono da parceira. Conclusão: O desamparo e a perda da autonomia da mulher em decidir sobre a continuidade da sua gestação torna essencial a elaboração de políticas públicas que incluam, cada vez mais, o homem no cenário reprodutivo. A abordagem dessa temática deve ser realizada de modo constante, nos espaços públicos e privados, pois somente o debate acerca da temática poderá contribuir no processo de humanização.AbstractIntroduction: Abortion is the product of conception eliminated in abortion. The decision to abort highlights the failure to carry out reproductive planning, domestic violence, parental abandonment, and the scarcity and/or supply of economic means. Objective: To analyze how men participate in the induced abortion process. Methodology: field study with exploratory qualitative approach, carried out in the rural area of the city of Antônio Gonçalves, Bahia. Through the snowball technique, five people were interviewed and the narratives were processed by the IRAMUTEQ Software that generated the Similitude Analysis. Results: The active participation and decisive influence of men in the induced abortion process is expressed through the offer of abortion method, the denial of paternity and the abandonment of the partner. Conclusion: The helplessness and loss of women’s autonomy in deciding on the continuity of their pregnancy makes it essential to develop public policies that increasingly include men in the reproductive scenario. The approach of this theme must be performed constantly, in public and private spaces, because only the debate about the theme can contribute to the process of humanization.

Implications of power imbalance in antenatal care seeking among pregnant adolescents in rural Tanzania: A qualitative study

Background Adolescent girls (10–19 years) are at increased risk of morbidity and mortality from pregnancy and childbirth complications, compared with older mothers. Low and middle-income countries, including Tanzania, bear the largest proportion of adolescent perinatal deaths. Few adolescent girls in Tanzania access antenatal care at health facilities, the reasons for which are poorly understood. Methods We conducted a qualitative thematic analysis study of the experiences of pregnant adolescents with accessing antenatal care in Misungwi district, Tanzania. We recruited 22 pregnant or parenting adolescent girls using purposive sampling, and conducted in-depth interviews (IDIs) about antenatal care experiences. IDI data were triangulated with data from eight focus group discussions (FGDs) involving young fathers and elder men/women, and nine key informant interviews (KIIs) conducted with local health care providers. FGDs, KIIs and IDIs were transcribed verbatim in Swahili. Transcripts were then translated to English and analysed using emergent thematic analysis. Results Four main themes emerged: 1) Lack of maternal personal autonomy, 2) Stigma and judgment, 3) Vulnerability to violence and abuse, and 4) Knowledge about antenatal care, and highlighted the complex power imbalance that underlies barriers and facilitators to care access at the individual, family/interpersonal, community, and health-systems levels, faced by pregnant adolescents in rural Tanzania. Conclusion Adolescent antenatal care-seeking is compromised by a complex power imbalance that involves financial dependence, lack of choice, lack of personal autonomy in decision making, experiences of social stigma, judgement, violence and abuse. Multi-level interventions are needed to empower adolescent girls, and to address policies and social constructs that may act as barriers, thereby, potentially reducing maternal morbidity and mortality in Tanzania.
