The incidence of inverted knee osteoarthritis is slowly increasing, there are technical limitations in the treatment, and the operation is difficult. In this article, we will study the benefits and costs of arthroscopic cleaning treatments based on intelligent electronic medicine. This article focuses on knee osteoarthritis patients in the EL database. There are 12 male patients, accounting for 66.67%, and 6 female patients, accounting for 33.33%. The average body mass index (BMI) of the patients was 28.08, the average time from first knee discomfort to surgery was 28.44 months, and the average time of arthroscopic debridement treatment for patients with VKOH knee osteoarthritis was 143.11 minutes. One case of perioperative complication occurred within 35 days after operation, which was a soleus muscle intermuscular venous thrombosis. After immobilization and enhanced anticoagulation for 1 week, it was stable without risk of shedding. The average postoperative study time was 20.00 months. The electronic medical arthroscopy cleaning treatment plan in this article can greatly improve the quality of life of patients and can check the pathological state in time, with low cost. In the course of treatment, comprehensive treatment costs can be saved by 45%. Arthroscopic clean-up treatment can not only reduce knee pain and other uncomfortable symptoms, restore normal knee joint function, and improve the quality of life of patients, but also correct the unequal length of the lower limbs, thereby avoiding spinal degeneration caused by knee instability. Therefore, it is the first choice for the treatment of advanced knee osteoarthritis in patients with VKOH.

Does medial patellofemoral ligament reconstruction result in femoral tunnel enlargement? A systematic review

Background Medial patellofemoral ligament (MPFL) reconstruction is a common surgical procedure for treating patellar instability. The primary aim of this systematic review was to determine whether MPFL reconstruction (MPFLR) leads to femoral tunnel enlargement (FTE). The secondary aims were to explore the clinical effects and risk factors of FTE. Electronic databases (MEDLINE, Global Health, Embase), currently registered studies, conference proceedings and the reference lists of included studies were searched independently by three reviewers. There were no constraints based on language or publication status. Study quality assessment was conducted. 3824 records were screened in the initial search. Seven studies satisfied the inclusion criteria, evaluating 380 knees in 365 patients. Rates of FTE following MPFLR ranged from 38.7 to 77.1%. Five low quality studies reported FTE did not lead to detrimental clinical outcomes as assessed with the Tegner, Kujala, IKDC, and Lysholm scores. There is conflicting evidence regarding change in femoral tunnel width over time. Three studies (of which two had a high risk of bias) reported age, BMI, presence of trochlear dysplasia and tibial tubercle-tibial groove distance did not differ between patients with and without FTE, suggesting these are not risk factors for FTE. Conclusion FTE is a common postoperative event following MPFLR. It does not predispose poor clinical outcomes. Current evidence lacks the ability to identify its risk factors. The reliability of any conclusions drawn is hindered by the low level of evidence of the studies included in this review. Larger prospective studies with long-term follow up are required to reliably ascertain the clinical effects of FTE.

Comparable Tibial Tunnel Widening despite Compound Differences of Bioabsorbable Interference Screws in Anterior Cruciate Ligament Reconstruction: a Retrospective Study

Purpose. To evaluate the effect of β-tricalcium phosphate (BCP) concentration in bioabsorbable interference screws on tibial tunnel widening after hamstring anterior cruciate ligament reconstruction. Methods. A total of 37 patients with a mean follow-up of 4.6 years were included for analysis. Bioabsorbable interference screws containing 60% BCP and 40% poly-L,Dlactic acid (PLDLA) (60BCP group) or bioabsorbable interference screws containing 30% BCP and 70% PLDLA (30BCP group) were used for tibial graft fixation. A 16-slice multislice computertomography scanner with post-process multislab reconstructions was used for tibial tunnel evaluation. The tibial tunnel diameter was measured at the midpoint of the tunnel. Tunnel widening was defined as an increase of more than 2 mm compared to the intraoperative drill diameter. Results. Patient demographics were not statistically different between both groups. The number of patients that showed tibial tunnel widening was high but did not differ statistically significant between the 60BCP and 30BCP group: 76% (n = 17) versus 75% (n = 20) for 60BCP and 30BCP group, respectively. The average increase in tibial tunnel diameter did not differ statistically significant between the groups; the tibial tunnel diameter increased with an average of 3.36 mm (SD 1.86) and 2.94 mm (SD 1.63) for the 60BCP and 30BCP group, respectively. Conclusions. A high incidence of tibial tunnel widening was observed when bioabsorbable interference screws containing different concentrations of β-tricalcium phosphate were used to fixate the hamstring graft in anterior cruciate ligament reconstruction. Increasing the concentration β-tricalcium phosphate did not result in less tunnel widening.
