Objective: to analyze the wearing of identification wristbands in newborns admitted in a public maternity hospital, regarding patient safety. Method: descriptive study, of the survey type, carried out in a reference public maternity hospital, through observations and interviews. Two hundred and sixty newborns were included. Results: 15.4% of the newborns had no identification wristbands, and 18% of the wristbands had data that did not match with the medical records. 90.9% of the wristbands were easily accessible for checking; however, in 80.9% of the cases, the wristband was not checked before the nursing procedures, and the mother or caregiver was not instructed on wearing the wristband in 76.8% of respondents. Conclusion and implications for practice: there should be training of the nursing team and other health professionals on the placement and daily checking of wristbands, considering international protocols and recommendations regarding patient safety.

Objetivo: Identificar as ações realizadas pela equipe de enfermagem, para alcançar a meta de segurança de identificação correta de pacientes, em uma unidade de internação psiquiátrica. Método: Estudo de caso, com abordagem qualitativa, realizado em uma unidade psiquiátrica de um hospital de ensino. A coleta de dados ocorreu mediante observação e entrevistas, com roteiro semiestruturado, com a equipe de enfermagem, e analisadas segundo a análise de conteúdo. Resultados: a identificação ocorria, por meio da admissão, uso de placas de identificação no leito, identificação no prontuário e uso de quadro branco no posto de enfermagem. O envolvimento da família melhora a identificação e a segurança do paciente, mas não houve identificação de uma prática baseada nos protocolos nem atuação do núcleo de segurança do paciente. Conclusão: Não havia uma sistematização no processo de identificação na unidade. Em razão das singularidades do pacientes psiquiátricos urge a necessidade de melhorias e identificação adequada.

Objective: to build a theoretical matrix of care system/organization of Nursing in a Neonatal Intensive Care Unit based on the meanings attributed by the Nursing Team professionals and other involved actors in the relationships and interactions of care in dealing with the fragility of living/surviving newborn. Method: a qualitative study adopting the theoretical framework the Complexity Paradigm and the Grounded Theory as research design. The participants were 22 subjects divided into four sample groups. Data was obtained from interviews and analyzed through comparative analysis, performed from May to August 2012. Results: nursing care practices management is based on the experience of the relationships/interactions with the family members and other health team actors, aiming to incorporate best practices. Thus, nursing care practices management seeks to address the fragility that surrounds prematurity, minimizing the sequelae when seeking newborn’s survival. Conclusion: the organization of nursing care system in a Neonatal Intensive Care Unit is complex, dynamic, interactive and interdependent with other health systems, mobilized by a management process of care and/or good care management practices that enables pre-term and his family a care focused on the fragility and uncertainties of living/surviving in singular conditions of life. The nurse stands out with the fundamental role of articulation in the light of the relationships/interactions in the care and organizational daily routine.
