Laparoscopic repair of abdominal wall hernias is increasingly used and widespread among surgeons, as it has shown advantages when compared to open repairs. Higher direct costs and the mandatory use of general anesthesia are arguments against its use, on the other hand, potential benefits aimed at improving quality of life include reduction in postoperative pain, quick recovery, lower recurrence rate, fewer complications and less overall cost, have already been proven. Although it was described about 30 years ago, the approach took a while to spread, possibly due to the anatomy that was still little explored, in addition to requiring advanced laparoscopic skills. The article in question reports the case of a patient with a direct inguinal hernia concomitant with the ipsilateral Spiegelian hernia, who received simultaneous repair of the two defects, via laparoscopy.

BACKGROUND: Laparoscopic approaches to inguinal hernia repair include transabdominal extraperitoneal and transabdominal preperitoneal, both of which are widely performed and employ mesh. Indicators of success for these surgical procedures include incidence of complications, time to return to daily activities, incidence of postoperative chronic pain, and the long-term postoperative patient satisfaction. OBJECTIVE: This study aimed to evaluate and compare long-term postoperative incidence of chronic pain and overall quality of life among patients undergoing transabdominal extraperitoneal or transabdominal preperitoneal inguinal hernia repair. METHODS: This was a retrospective cross-sectional study. Medical records were analyzed, and the SF-36 questionnaire and Visual Analog Scale were applied to assess quality of life and chronic pain in patients undergoing laparoscopic inguinal hernia repair between January 2017 and February 2021. RESULTS: A total of 167 patients status post laparoscopic inguinal hernia repair, who were 3 months postoperatively or longer, were included in the study. Among the early complications seen, seroma was most common in the transabdominal preperitoneal group (p=0.04). Subsequently, 40 of the initial 167 patients answered to the survey instrument (SF-36 and Visual Analog Scale). Mean patient-reported pain (Visual Analog Scale score) was statistically similar between groups, with 1.29 for transabdominal preperitoneal and 1.68 for transabdominal extraperitoneal (p=0.92). In the domains evaluated by the SF-36, there was no significant difference between the samples. CONCLUSION: Both transabdominal extraperitoneal and transabdominal preperitoneal techniques for hernia repair have similar results in the late postoperative period regarding quality of life and prevalence of chronic pain. They are also comparable in terms of major early postoperative complications, except for seroma, with a higher incidence in patients undergoing transabdominal preperitoneal.

As hérnias são protrusões anormais de órgãos, frequentemente localizadas na região abdominal, sendo as inguinais as mais comuns, representando cerca de 75% de todos os casos. A região inguinal é crucial para compreender as hérnias, com características anatômicas complexas, incluindo canais e anéis específicos. As hérnias inguinais podem ser classificadas como diretas ou indiretas, dependendo da sua localização em relação aos vasos epigástricos inferiores. As diretas resultam do enfraquecimento da fáscia transversal, enquanto as indiretas estão relacionadas a um processo vaginal persistente, muitas vezes congênito. Essas hérnias também podem ser primárias ou recorrentes, unilaterais ou bilaterais, congênitas ou adquiridas e, ainda, podem apresentar complicações como encarceramento e estrangulamento. Os sintomas incluem dor na região inguinal, especialmente durante esforços físicos, tosse ou ao ficar em pé. O diagnóstico é predominantemente clínico, mas exames de imagem como ultrassonografia e ressonância magnética podem ser úteis em casos complicados ou de hérnias recorrentes. No aspecto do tratamento cirúrgico, as intervenções abertas podem ser conduzidas utilizando técnicas com o emprego de telas, que buscam minimizar a tensão, ou por meio de procedimentos que envolvem a correção dos tecidos, com reconstrução do assoalho inguinal utilizando tecido do próprio paciente. Por outro lado, a técnica videolaparoscópica é bem menos invasiva, com as seguintes vantagens: menos dor após a operação, recuperação mais rápida para retomar as atividades habituais e maior satisfação por parte dos pacientes. Por isso, foi elaborada uma revisão sistemática da literatura, examinando cuidadosamente estudos e pesquisas científicas publicados nos últimos 10 anos, acessando as bases de dados PubMed e LILACS. Este processo nos permitirá avaliar as duas modalidades cirúrgicas disponíveis, bem como destacar qual técnica apresenta os resultados mais satisfatórios.
