Abstract: Background - Biliary complications (BC) represent the most frequent complication after liver transplantation, up to 34% of cases. Aim: To identify modifiable risk factors to biliary complications after liver transplantation, essential to decrease morbidity. Method: Clinical data, anatomical characteristics of recipient and donors, and transplant operation features of 306 transplants with full arterial patency were collected to identify risk factors associated with BC. Results: BC occurred in 22.9% after 12… Show more

“…While the relevance of CMV detection and prevention after LT has been largely established, the association between CMV infection and ABS development remains unclear ( 20 22 ). Among the large existing body of literature focused on ABS risk factors, very few series have found similar findings ( 23 25 ). The current study was not designed to investigate mechanisms of CMV on ABS occurrence, but one can hypothesize that destruction of vascular endothelial cells due to CMV infection might lead to arterial thrombosis, resulting in biliary ischemia ( 26 ).…”

“…Yet, in the current study, this failed to reach statistical significance upon multivariable analysis. However, a tiny duct size has been already reported as an ABS risk factor in multiple series including one randomized trial ( 25 , 31 ). In order to overcome the technical difficulty, using ductoplasty techniques or T-tube insertion was at the discretion of the surgeon.…”

“…It is important to assess locoregional lymph nodes in order to avoid LT in cases with positive lymph node disease 11 , 15 . Just as in the protocols performed at the Mayo Clinic 16 , 20 , there is the suggestion of performing laparoscopy or laparotomy with evaluation of the lymph node chains mentioned above (8a, 8p, 12a and 12p).…”

“…Nas últimas duas décadas houve progresso na sobrevida após TH em 20-30%, melhorias nos exames de imagem e introdução de imunossupressores com ação antineoplásica (inibidores do mTOR) 15,17 . Esta evolução técnica aliada ao peculiar panorama de transplantes na Noruega, onde há mais doadores de órgãos do que receptores em lista, proporcionou o cenário ideal para retomar a utilização do TH nas MHCCR.…”

“…In the past two decades, there has been improvement in the survival rates after LT by 20-30% and improvement in the imaging examinations; there was also the introduction of immunosuppressants with antineoplastic action (mTOR inhibitors) 15 , 17 . This technical progress combined with the peculiar transplantation scenario in Norway, which has more organ donors than recipients on the list, provided the ideal scenario for performing LT in CRLM.…”
