Adjuvant Chemoradiotherapy After Subtotal or Total Gastrectomy and D2 Limphadenectomy Increases Survival in Advanced Gastric Cancer?

Abstract: Background:The treatment of advanced gastric cancer with curative intent is essentially surgical and chemoradiotherapy is indicated as neo or adjuvant to control the disease and prolong survival. Aim:To assess the survival of patients undergoing subtotal or total gastrectomy with D2 lymphadenectomy followed by adjuvant chemoradiotherapy. Methods:Were retrospectively analyzed 87 gastrectomized patients with advanced gastric adenocarcinoma, considered stages IB to IIIC and submitted to adjuvant chemoradiotherapy… Show more

“…Complications were frequent (47%) a higher when compared to series with younger patients 16 , 17 . Major complications occurred in 21.5% of the patients and were usually due to clinical ones, which is similar to what other authors observed 2 , 11 , 12 , 14 . Patients with POC had longer hospital length of stay and shorter OS.…”

“…Laparoscopic surgery has proved to be safe and effective with less pain, less bleeding and a shorter recovery time. The randomized studies during the last decade have shown excellent results in terms of complication rates, which are very similar to open surgery 1,2,20,21,28,29 .…”

“…In terms of oncological safety, the number of lymph nodes and resected lymph nodal barriers do not differ in any way from open surgery 1,24 . Moreover, it is possible that lymph node dissection becomes more precise and less difficult with 3D laparoscopic or robotic surgery (R0 oncological surgery with better survival) 22,24,30 .…”

“…G astric cancer (GC) has high prevalence worldwide and is a major cause of cancer-related death 2,10,20 . Lymph node metastasis is the most important prognostic factor 18,23,29 ; however, nearly 10% of pN0 patients who underwent gastrectomy have disease recurrence 18,22,23,29 .…”
