China has established a comprehensive primary medical health service system, but the development of primary medical health services in the central and western regions is still unbalanced and insufficient. Based on data from 2010 to 2019, this paper constructs a super efficiency Slack-Based Measure model to calculate the supply efficiency of primary medical health services in 20 provinces and cities in central and western China. Using Kernel density estimation and Markov chain analysis, this paper further analyzes the spatial-temporal evolution of the supply efficiency of primary medical health services in central and western China, and also predicts the future development distribution through the limiting distribution of Markov chain to provide a theoretical basis for promoting the sinking of high-quality medical resources to the primary level. The results show that firstly, during the observation period, the center of the Kernel density curve moves to the left, and the main peak value decreases continuously. The main diagonal elements of the traditional Markov transition probability matrix are 0.7872, 0.5172, 0.8353, and 0.7368 respectively, which are significantly larger than other elements. Secondly, when adjacent to low state and high state, it will develop into convergence distributions of 0.7251 and 0.8243. The supply efficiency of primary medical health services in central and western China has the characteristics of high (Ningxia) and low (Shaanxi) aggregation respectively, but the aggregation trend is weakened. Thirdly, the supply efficiency of health services has the stability of keeping its own state unchanged, but the transition of state can still occur. The long-term development of the current trend cannot break the distribution characteristics of the high and low clusters, the efficiency will show a downward trend in the next 10–20 years, and still the problem of uneven long-term development emerges.

Background Healthcare services efficiency (HSE) is directly related to the healthcare demands of the general public and also plays an essential role in the country’s coordinated economic and social development. Methods In this study, the stochastic frontier approach (SFA)-Malmquist model was applied to measure the HSE of 31 Chinese provinces based on panel data from 2010–2020. Then, kernel density estimation, Markov chain, and exploratory spatial data analysis were adopted to study the temporal-spatial dynamic evolution characteristics of the HSE. Results The study found that China’s HSE showed an average value of approximately 0.841, indicating room for improvement. The HSE varied significantly across regions, presenting an “East > Central > West” distribution layout. The TFP of healthcare services in China grew by 1.6% per year, driven mainly by technological progress of 1.8% per year. The trend of the HSE shifting to a high level in China was significant, but its evolution exhibited stability of maintaining the original state, and it was harder to achieve leapfrog transfer. The temporal-spatial evolution of the HSE was also significantly affected by geospatial factors, with a clear spatial spillover effect and spatial agglomeration characteristics. Provinces with high-level HSE exhibited positive spatial spillover effects, while provinces with low-level HSE had negative spatial spillover effects. Thus, the “club convergence” phenomenon of “high efficiency concentration, low efficiency agglomeration, high levels of radiation, and low levels of suppression” was formed in the spatial distribution. Conclusions The results indicate that countermeasures should be taken to improve the HSE in China. Theoretical support for the improvement of HSE is provided in this paper.

This study targets six North Asia and African hospitals that provide clinical treatment, diagnostic, and consultation services. The study revealed a lack in the selected hospitals' infrastructure to support implementation successfully Environmental Management Systems (EMS). A factor analysis was performed based on five factors; EMS efficiency, the hospital's culture, support from top management, legislation, and time and budget controls. Out of 23 sub-factors tested, it has been found that 7 of them can control the differentiation of the factors. The results highlight human errors in chemical and medical materials that lead to environmental pollution. It was followed by a sub-factor that indicated that controls of hospital activities and correction of its performance depend on the feedback received from the environmental information system. The study's main conclusion is that recovering green hospital systems depends on successfully carrying out the ISO 14001: 2015 standard.
