Herniorrhaphy is one of the most common surgical procedures, with an estimated 20 million operations performed annually worldwide. One of the common complications associated with this procedure is inguinodynia, defined as pain beyond three months after inguinal hernia surgery. In this review, we have addressed the main aspects of this complication with current articles, published in the last five years. Inguinodynia has a multifactorial nature and studies have shown that its development is related to the surgical technique and intrinsic factors of the patient that imply greater predisposition to this phenomenon. In this regard, it has been discussed which surgical techniques imply a lower incidence of this complication. Many studies have focused on understanding intrinsic features of each patient, both in physical and cognitive aspects, and how the approach of these factors can favor a better post-surgical recovery. The treatment of this condition is still challenging, and there are no established universal guidelines. We believe that due to its multifactorial nature, the treatment is hampered due to the individuality inguinodynia presentations.

Nerve identification in open inguinal hernioplasty: A meta-analysis

Objective: In open inguinal hernioplasty, three inguinal nerves are encountered in the surgical field. It is advisable to identify these nerves as careful dissection reduces the chances of debilitating post-operative inguinodynia. Recognizing nerves during surgery can be challenging. Limited surgical studies have reported on the identification rates of all nerves. This study aimed to calculate the pooled prevalence of each nerve from these studies. Material and Methods: We searched PubMed, CENTRAL, CINAHL, ClinicalTrials.gov and Research Square. We selected articles that reported on the prevalence of all three nerves during surgery. A meta-analysis was performed on the data from eight studies. IVhet model from the software MetaXL was used for preparing the forest plot. Subgroup analysis was performed to understand the cause of heterogeneity. Results: The pooled prevalence rates for Ilioinguinal nerve (IIN), Iliohypogastric nerve (IHN), and genital branch of genitofemoral nerve (GB) were 84% (95% CI 67-97%), 71% (95% CI 51-89%) and 53% (95% CI 31-74%), respectively. On subgroup analysis, the identification rates were higher in single centre studies and studies with a single primary objective as nerve identification. The heterogeneity was significant in all pooled values, excluding the subgroup analysis of IHN identification rates in single-centre studies. Conclusion: The pooled values indicate low identification rates for IHN and GB. Significant heterogeneity and large confidence intervals reduce the importance of these values as quality standards. Better results are observed in single-centre studies and studies which are focused on nerve identification.

Introduction: Inguinodynia, is a frequent problem in clinical practice. The need to improve its assessment is obvious. Currently, there is a lack of agreement regarding the appropriate way to evaluate this syndrome. The objective of this paper is to illustrate and propose a modification of the dermatome mapping (DM) tool to improve its efficacy, by combining it, using the analogous verbal numerical scale of pain (VANSP) to identify the affected nerves from the dermatome involved, to objectively evaluate pain intensity and to assess the treatment given. Methods: We use the analogous verbal numerical scale of pain (VANSP) to mark the patient’s skin, applied to the method of DM described by Álvarez. We ask the patient to characterize the pain intensity with numbers, beginning with one, for the slightest pain, to 10 for the worst possible pain experienced. After the treatment is given, either if it is conservative, surgical, or neural block, we repeat the mapping to evaluate its effectiveness. Results: We have used this approach since 1997, in several patients with inguinodynia of different etiologies pre- and post-treatment, and it is useful and a more objective evaluation, especially to estimate the result of the treatment given. Conclusion: Dermatome mapping, marking the patient pain points using the VANSP, can provide relevant information for the diagnosis, treatment, and assessment of management results in patients with inguinodynia.
