O câncer de mama configura uma questão de saúde pública mundial, pois é o tumor com a maior incidência e mortalidade na população feminina, em todo o mundo. No Brasil ocorrem em torno de 58 mil casos novos de câncer de mama anualmente, com incidência estimada de 56,20 casos a cada 100 mil mulheres. Raro em jovens, porém extremamente agressivo nesta faixa etária. A imunoistoquímica classifica esses tumores em cinco padrões moleculares distintos: luminal A luminal B, triplo negativo, super expressão do receptor do fator de crescimento epidérmico humano 2 (HER-2) e os não classificáveis. Estes tipos biológicos apresentam diferenças importantes quanto as suas próprias características e as características individuais da paciente, sendo aqueles triplos negativos, os de pior prognóstico. Esse trabalho tem como objetivo relatar a evolução de um câncer de mama inicialmente inoperável que apresentou progressão tumoral durante a quimioterapia neoadjuvante, em uma paciente de 33 anos. O tumor ocupava toda a mama direita, com comprometimento da pele e da axila direita (T4d N1 Mx, estádio clínico IIIB). Como houve falha no tratamento clínico inicial, optou-se pela radioterapia neoadjuvante de resgate, a qual proporcionou visível redução global da lesão, permitindo a realização da mastectomia radical com linfadenectomia da região axilar. Portanto, o intercâmbio entre as modalidades terapêuticas (cirurgia, quimioterapia e radioterapia) deve ser sempre lembrado e ajustado a cada caso, a fim de proporcionar uma melhor abordagem terapêutica individualizada.
