Ivory vertebra: imaging findings in different diagnoses

Abstract: Low back pain is often managed at all levels of healthcare. In general, diagnostic investigation begins with radiography of the lumbar spine. In addition to the most common findings, radiologists can identify increased density of a vertebral body, referred to as ivory vertebra. The objective of this study was to describe the main diseases that can present with this radiologic sign, such as Hodgkin lymphoma, Paget's disease, metastatic prostate cancer, breast cancer, and osteomyelitis. It is extremely important… Show more

“…Several, but rare, complications have also been noted in some patients, most commonly being the development of osteosarcoma (81%), fibrosarcoma (14%), chondrosarcoma (3%), malignant fibrous histiocytoma (1%), malignant giant cell tumour and lymphosarcoma (0.5%) 14 15. No treatment was given to the patient in our Dental College; he was referred to a higher centre for better management and treatment.…”

“…Skeletal abnormalities in TSC include cyst-like lesions, hyperostosis of the inner table of the calvaria, osteosclerotic changes ( Figure 11 ), periosteal new bone formation, cystic changes in the phalanges, and scoliosis ( 3 ) . These bone abnormalities can also be seen in several other diaseases ( 17 - 22 ) , and only in the presence of changes in other organs that fulfill TSC criteria can be associated with the complex. Retinal hamartomas are present in about 40-50% of patients with TSC.…”

“…Although osseous lymphoma was classically considered in the differential diagnosis of an ivory vertebra, this is a relatively rare occurrence. 33,34 The mixed sclerotic-lytic type is more common in secondary lymphomas and after treatment. 30 However, radiographic findings may be very subtle even in the presence of aggressive appearance on other imaging modalities (►Fig.…”
