Molecular identification of Sporothrix species involved in the first familial outbreak of sporotrichosis in the state of Espírito Santo, southeastern Brazil

Abstract: Sporotrichosis is a widespread subcutaneous mycosis caused by the dimorphic fungi now known as the Sporothrix schenckii complex. This complex is comprised of at least six species, including Sporothrix albicans, Sporothrix brasiliensis, Sporothrix globosa, Sporothrix luriei, Sporothrix mexicana and S. schenckii. Cases of sporotrichosis have significantly increased in Brazil over the past decade, especially in the state of Rio de Janeiro (RJ), where an epidemic among cat owners has been observed. The zoonotic tr… Show more

“…This information would be important in view of the emerging changes in the epidemiological profile of the disease in the last decades in Brazil. 11,[14][15][16] However, although we have reported few autochthonous cases of zoonotic sporotrichosis caused by S. brasiliensis, related to contact with cats in the state of Espírito Santo (some of which were included in this study), 20,21 our results suggest the occurrence of the "classic disease transmission pattern," probably caused by S. schenckii s.s. 22 between 1982 and 2012.…”

“…Some of these cases were partially described in previous publications. [20][21][22][23] The majority of the patients were male (80.7%, 4:1 male-to-female ratio); most patients were Caucasian (86.5%), and showed an age range from 6 to 77 years. Only six patients (3.5%) were younger than 12 years old ( Table 1).…”

“…19 S. brasiliensis, the most virulent species of the complex, 4 has frequently been reported in infections in Brazil, often affecting immunocompetent patients. Case reports of lung injury in HIV negative patients, 28 a familial outbreak in the southeastern of Brazil, 27 and even resistance to itraconazole treatment in animals have been reported. 12 Sporothrix species are also the most frequent pathogens in cases of osteoarticular infection in immunocompetent patients.…”

“…In all cases, cats seemed to play an important role in the transmission of the disease: patients had contact with cats with sporotrichosis and more than one family member presented lesions simultaneously. Specifically, Ss368, Ss382 and Ss383 (cat) corresponded to well‐documented cases of zoonosis . The patient (Ss380) was a woman, originally from Rio de Janeiro.…”
