Detection of HIV-1 infections in blood donors during the pre-seroconversion window period in São Paulo, Brazil

Abstract: By decreasing the pre-seroconversion window period, nucleic acid testing (NAT) has improved the safety of blood products and reduced the risk of transfusion-transmitted infections. Between 2011 and 2017, NAT determinations for approximately 898,202 donations were performed at Fundação Pró-Sangue/Hemocentro de São Paulo (FPS-HSP). Three seronegative HIV-viremic donations were detected. The NAT yield rate per million donations was 3.34 for HIV, and the acute HIV-1 infections detected are described, followed by a… Show more

“…Using molecular testing, we reported a high frequency of HIV, HBV and HCV in blood donors approved for blood donation by the routine screening process in two blood banks. For HIV and HCV, this escape from the serological screening process may be linked to the diagnostic window period as explored in several studies [ 24 , 25 ]. For HBV, beyond the window period, the non-detection of surface antigen (HBsAg) may be linked to occult Hepatitis B infections [ 26 28 ].…”

“…At FPS-HSP, NAT for HIV and HCV started in 2011, two years before it became mandatory in the country. Donations during this window period revealed an HIV NAT positivity rate of 3.60 per 1,000,000 donations between 2011 and 2020 [ 22 ] that further prevented transfusion-transmitted HIV [ 31 ].…”

“…In our center, one of the largest blood banks in Latin America, approximately 110,000 units of blood are collected annually from non‐remunerate volunteer donors. In 2020, 2% of donated blood was discarded following infectious disease screening, with 0.11% due to the detection of HIV‐related biomarkers 20 . The objective of the present study was to determine the frequency of undeclared ARV drug use in blood donations reactive for HIV markers by direct testing for ARV drugs.…”
