First case report of cryptococcosis due to Cryptococcus decagattii in a pediatric patient in Argentina

Abstract: We report the first case of cryptococcosis due to Cryptococcus decagattii in an immunocompetent pediatric patient from an indigenous community in Argentina with a successful outcome. Two isolates (blood, cerebrospinal fluid) were genotyped by restriction fragment length polymorphism of the orotidine monophosphate pyrophosphorylase (URA5) gene as VGIV and identified by multi-locus sequence typing as C. decagattii. Matrix-assisted laser desorption/ionization time of flight mass spectrometry identification indica… Show more

“…It was first named genotype VGIIIc 3,49 but recently renamed VGVI 50,51 . Recently, a case report of cryptococcosis caused by C. decagattii in Argentina was published in a patient belonging to a community living in huts made of branches in close physical contact with nature and domestic animals 48 . Our results are surprising because cases of cryptococcosis due to C. tetragattii are rare and even more common in Europe, and we described a new ST 556.…”

“…Among the VGVI strains, we identified one C. decagattii strain, serotype B, which came from Spain. C. decagatti has rarely been isolated, and there are few reports worldwide 3,48 . It was first named genotype VGIIIc 3,49 but recently renamed VGVI 50,51 .…”

“…In the literature, less than a thousand cases of cryptococcal infection have been reported in children worldwide; in Latin America, no more than 50 cases have been described [32][33][34]. Caused by both C. neoformans and C. gattii species complexes, cryptococcosis cases in otherwise healthy and HIV-infected children has been documented in Argentina [35], Bolivia [35], Brazil [36,37] and Colombia [33,38]. In the case reported here, a VNII isolate (ST43) was recovered from the 9-year-old Peruvian girl.…”

“…The remaining two isolates were selected for sequencing from the Westmead Medical Mycology Collection, Sydney/Perth, Australia due to their MLST profiles. These were collected from a clinical case in Argentina [7] and a veterinary case in Arizona (Table 1). *The isolate was obtained from a Mexican non-HIV patient living in Spain Genome libraries were prepared with a DNA prep kit (Illumina) at 1/4 th of the reaction volumes and sequenced on a NextSeq 550 (Illumina).…”
