Social determinants of mortality due to visceral leishmaniasis in Brazil (2001-2015): an ecological study

Abstract: Introduction: We aimed to analyze the relationship between visceral leishmaniasis mortality and social determinants of health (SDH). Methods: This was an ecological study of all leishmaniasis-related deaths in Brazil, from 2001 to 2015. We analyzed 49 indicators of human development and social vulnerability. The association was tested using the classical and spatial regression model. Results: Mortality was associated with indicators that expressed low human development and high social vulnerability: lack of ga… Show more

“…The Human Development Index (HDI) was used as an indicator for understanding the socioeconomic profile of the municipalities of São Paulo state. The HDI is a well-known and comparable indicator worldwide, reproducing studies in other states and regions countrywide [17]. The HDI value of the 45 municipalities in the RHN11 was obtained in Fundação Sistema Estadual de Análises de Dados (SEADE), from 1999 to 2016, coded and classified by the denominated quintile as a reference parameter.…”

“…Depending on the region, Leishmania is transmitted by different phlebotomine sand fly vectors ( 3 ). There is no human vaccine ( 4 , 5 ) and the prevalence of leishmaniasis is closely linked to the human development index and environmental degradation ( 6 , 7 ). This zoonosis remains a serious public health problem and is recognized as one of the 20 major neglected tropical diseases ( 8 ).…”

“…Moreover, social determinants have been related to VL mortality, especially in the North and Northeast regions. Recent ecological study highlighted high mortality rates of VL associated with unplanned urbanization and precariousness of households, where both reservoirs and breeding sites for disease vectors are present [ 59 ].…”

“…Additionally, we recognize that the SVI can be an important tool for future health research in Brazil because it is a robust and multidimensional index developed for the purpose of guiding decision making of public managers and help researchers to better understand the different aspects of social vulnerability in Brazil [ 14 ]. Some studies applied SVI or its indicators in spatial analysis of leprosy [ 28 , 62 , 68 ] and VL mortality [ 59 ], but they showed promising insights for using the index in epidemiological research.…”
