A case of chikungunya virus disease presenting with remarkable acute arthritis of a previously damaged finger joint

Abstract: Chikungunya virus (CHIKV) is a mosquito-borne arthritogenic alphavirus that has recently been introduced to Brazil. We report the case of a 36-year-old male patient from the City of Rio de Janeiro who developed molecularly-confi rmed CHIKV disease and whose clinical picture was remarkable because of acute arthritis of an interphalangeal joint that had been damaged by trauma 8 years previously. This case illustrates that acute CHIKV disease may preferentially target previously damaged joints. Careful study of i… Show more

“…Weakness in the hands during acute illness was highly associated with the risk of persistent arthralgia in our study participants. Previous studies of chronic CHIKV disease patients have demonstrated joint space narrowing in the distal joints upon radiographic imaging [ 43–45 ] and remarkable persistent arthritis of these joints [ 46 ], suggesting that disseminated infection and inflammation during acute illness increases the likelihood of persistent arthralgia symptoms. Clinical management of acute symptoms, particularly inflammation, and the minimization of acute disease duration could reduce the incidence, morbidity, and economic impact of chronic CHIKV disease.…”

“…The case reports and case series found nonspecific but important joint alterations. One case report by Eyer-Silva et al 17 described a 36-year-old man infected with CHIKV who developed severe acute arthritis in a finger joint previously damaged by trauma, confirmed on radiography.…”

“…( 16) Global warming/climate change, globalization with signi cant increase in international travels, and adaptation of virus to new vectors has also increased the vector susceptibility and transmission capacity. (13,19) CHIKV most probably rst emerged as a human pathogen in the 18th century, but currently has been identi ed in nearly 80 countries across 5 continents. (10) Most epidemics of chikungunya fever occurred in tropical or subtropical areas; Eritrea's subtropical climate is suitable for Aedes mosquitoes and the transmission of mosquito-borne diseases, such as CHIKF.…”

“…(2,(10)(11)(12) CHIKV, like the dengue and zika virus, are commonly classi ed as arthritogenic viruses as these viruses cause musculoskeletal in ammatory disease in humans. (13) Upon infection, CHIKV has an incubation period of 3-7 days, but may last as long as fourteen days. (6, 14) Seroprevalence studies have demonstrated that 30-40% of CHIKV infected individuals can be asymptomatic, but the majority (60-80%) of infected individuals are symptomatic.…”
