Early reperfusion therapy in acute ischemic stroke after recent myocardial infarction

Abstract: A 50-year-old woman with recent myocardial infarction (MI), presented right hemiparesis with aphasia (National Institutes of Health Stroke Scale: 18), 24 hours after the coronary angiogram that revealed a large apical thrombus ( Figure 1A). Computed tomography demonstrated left middle cerebral artery (MCA) "dot" sign ( Figure 1B). Angiography showed occlusion in the MCA second segment ( Figure 1C) with recanalization after mechanical thrombectomy ( Figure 1D). She presented a complete recovery (Rankin: 0) and … Show more

“…The incidence rate of IS is high in the elderly, and the disease severely affects the prognosis of MI patients ( 3 ). MI patients complicated with IS have a significantly higher mortality rate than patients with MI alone ( 4 , 5 ), which has attracted extensive attention in the clinic. Studies have shown that there are often abnormal ECG changes of patients with acute IS, and ECG changes of these patients are very sensitive with a very low specificity, suggesting that ECGs are insufficient to be used as a diagnostic criterion for IS ( 6 ).…”

“…The stent retriever is also an alternative when thrombolytics are contraindicated, such as after a myocardial infarction within the previous three months 22 . The stent retriever has the potential risk of arterial dissection, muscular spasm and endothelium lesion and this risk increases with the number of passes over the artery.…”

“…The Garcia scale uses 6 tests to assess the asymmetry of the animal’s movements and reactions in points (from 1, pronounced impairment, to 3, no impairment), which are also then summarized. The minimum neurological result is 3 points (maximum impairment), and the maximum is 18 points (no impairment) [ 55 ].…”
