Although substantial increases in the numbers of elderly people are now foreseen in all countries, greater growth is expected in developing regions such as Brazil, where the proportions are expected to become 18.8% in 2020 and 29.3% in 2050. 1,2 Primary care is considered to be the front line for healthcare for the elderly and can provide regular contacts focused on preventing disabilities resulting from chronic health conditions, such as classification of cognitive impairment in this age group. 3 Healthcare professionals are faced with the challenge of evaluating the limit of normality among elderly people's cognitive alterations. Within the concept of senescence, they need to differentiate the expected changes for this age group from the pathological conditions of aging that constitute senility. If such conditions are seen at the prodromal stage, reversal or mitigation may still be possible. 4,5 Development of dementia in elderly people is a measurable risk. Thus, the pathological transition to this, from a mild stage of cognitive impairment, forms a "gray zone" between normality and initial dementia. 4 Screening for cognitive impairment among elderly people can be achieved through instruments that have already been translated and validated for application in Brazil. 6 Bustamante et al. 7 suggested that cognitive tests and functional scales should be used in combination, in populations with educational heterogeneity. The objective of this study was to investigate the impact of age and schooling on Brief Cognitive Screening Battery (BCSB) performance in elderly residents in the city of Rio de Janeiro, Brazil. The sample was composed of 470 elderly individuals, all of whom were evaluated using the BCSB. The results were analyzed by groups that were divided by age and schooling. Analyses of variances (ANOVAs) were performed to examine differences in performance between the age and schooling groups and interactions between age and schooling. Correlations between cognitive variables, age, and schooling were also analyzed. The sample consisted of 420 women (89.4%) and 50 men (10.6%). The mean age was 72.77 Ϯ 7.06 years, with average schooling of 9.54 Ϯ 5.32 years. The ANOVA showed that elderly individuals who were older than 80 years of age presented worse performance on memory tasks compared with the younger age groups. With regard to schooling, elderly individuals with low schooling presented worse performance on the Verbal Fluency and Clock Drawing tests and on the Mini Mental State Examination. The two-way ANOVA revealed a significant Age ϫ Schooling interaction for Mini Mental State Examination scores. The Age ϫ Schooling interaction impacted performance of the elderly in the evaluation of global cognitive function. The present results indicated a dissociation, in which age affected memory function, and schooling affected other cognitive functions.

Summary Evidence linking obstructive sleep apnea with cognitive dysfunction predominantly comes from clinical or select community samples. We investigated the independent cross‐sectional association of obstructive sleep apnea and sleep macroarchitecture parameters with cognitive function in unselected community‐dwelling middle‐aged and older men. Four hundred and seventy‐seven Florey Adelaide Male Ageing Study participants underwent successful home‐based polysomnography. They also completed cognitive testing, including the inspection time task, Fuld object memory evaluation, trail‐making test A and B, and mini‐mental state examination. Multivariable regression models examined independent cross‐sectional associations of obstructive sleep apnea and sleep macroarchitecture parameters with cognitive function. In univariable analyses, a higher apnea–hypopnea index and percentage of total sleep time with oxygen saturation <90% were associated with worse trail‐making test A performance (both p < .05). A higher apnea–hypopnea index was also associated with worse trail‐making test B performance and slower inspection time (both p < .05). In adjusted analyses, obstructive sleep apnea and sleep macroarchitecture parameters were not associated with cognitive function (all p > .05). In age‐stratified analysis in men ≥65 years, greater stage 1 sleep was independently associated with worse trail‐making test A performance, whereas greater stage 3 sleep was independently associated with better trail‐making test A performance (both p < .05). Our findings suggest that obstructive sleep apnea is not independently associated with cognitive function. In older, but not younger, men, light sleep was associated with worse attention, whereas deep sleep was associated with better attention. Longitudinal population‐based cohort studies are needed to determine if obstructive sleep apnea and disrupted sleep macroarchitecture independently predict prospective cognitive dysfunction and decline.
