Update on Idiopathic Intracranial Hypertension Management

Abstract: Background: Idiopathic Intracranial Hypertension (IIH) is a secondary headache with a steadily growing incidence. Currently, there is little evidence to guide the treatment of IIH. Objective: To review the pathophysiology of IIH, with focus on the role of obesity as a risk factor, and the implications for new therapeutic perspectives. Methods: in this narrative review, we summarized the current knowledge on treatment options highlighting available evidence for managing intracranial hypertension, obesity, an… Show more

“…Other nonendovascular treatment options for restenosis after VSS include CSF diversion, optic nerve sheath fenestration for patients with significant visual difficulty with evident optic nerve compression, bariatric surgery, and additional medical treatment. 50 We have noted elevated intrathoracic pressures (measured in the superior vena cava) in a subset of patients with IIH. These elevated pressures many times exist without the presence of an ICP gradient and are always associated with morbid obesity.…”

“…It is associated with a low complication rate and high technical success. 24,25 A study that evaluated TVSS after spontaneous CSF leak closure surgery in patients with IIH showed that it reduces CSF leak recurrence and is well tolerated. 26 TVSS could also be considered preoperatively in patients with spontaneous CSF leaks to reduce the need for surgery.…”
