Diffuse alveolar hemorrhage in childhood-onset systemic lupus erythematosus: a severe disease flare with serious outcome

Abstract: Objective: To evaluate prevalence, clinical manifestations, laboratory abnormalities and treatment in a multicenter cohort study including 847 childhood-onset systemic lupus erythematosus (cSLE) patients with and without diffuse alveolar hemorrhage (DAH), as well as concomitant parameters of severity. Methods: DAH was defined as the presence of at least three respiratory symptoms/signs associated with diffuse interstitial/alveolar infiltrates on chest x-ray or high-resolution computer tomography and sudden dro… Show more

“…Moreover, 94.4% of the children with pulmonary involvement display bilateral presentation. The high prevalence of bilateral involvement in our study is in line with the previous studies (1,9,12). The presence of anti-RNP antibody was described to be specific (specificity ranging from 84 to 100%) of mixed connective tissue disease (13).…”

“…Results on SLE-associated DAH from previous studies from 2009 to 2019 are summarized in Table 4. Prevalence in our cohort was close to the ones observed in other studies, 1,2 and similar in childhood-onset SLE, 3,4 but higher than the one reported by us 9 years ago and by some other older series 5,6 (Table 4). This may be explained by a lack of recognition of DAH as SLE manifestation, especially when DAH occurred in the early course and systematic screening was not done.…”

“…In a more recent report, there were 13/19 SLE patients who had their DAH occurring within the first year after SLE diagnosis. 3 Of note, even though DAH tends to occur early, it still can present as late as 3 years after SLE diagnosis. Thus, DAH should be suspected in all SLE patients with suspicious symptoms or signs, especially in the first 3 years after SLE diagnosis.…”
