Introduction: To reduce the risk of insulin resistance in obesity in children with lactase gene genotypes, we studied the factors that stimulate the chronic inflammatory process. Material and methods: 109 children 6–18 years of age were investigated. The main group (n = 56) was presented by children with signs of insulin-resistant obesity according to the criteria of the European Society of Endocrinology and the Pediatric Endocrine Society. The control group (n = 53) included obese children without insulin resistance. A comprehensive clinical examination, food diary analysis, genotyping of the lactase gene by means of the polymerase chain reaction, the Immunochemical Test Method with Electrochemiluminescent Detection of basal insulinemia, Hydrogen breath test with lactose load, sequential analysis, ROC analysis were carried out. Results: Clinical manifestations of lactose maldigestion in a child increased the risk of possible insulin resistance (prognostic coefficient (PC +2.6), as well as the presence of the lactase C/C 13910 gene genotype (PC +5.8) did. The genotype C/T 13910 in children had a protective effect on the risk of obesity (PC −2.9). The lowest risk of insulin-resistant obesity in observed among children with the genotype T/T 13910 (PC −12). Conclusion: The presence of the C/C 13910 genotype of the lactase gene is the main factor formation of insulin resistance in children’s obesity. What is known? The genotype C/C 13910 of the lactase gene as a risk factor for the chronic inflammatory process in the body. What is New? Genotype C/C 13910 of the lactase gene as a risk factor for insulin-resistant obesity in children.

Circulating Spexin Decreased and Negatively Correlated with Systemic Insulin Sensitivity and Pancreatic β Cell Function in Obese Children

Objectives: Spexin (SPX) is a novel peptide that has recently emerged as an important regulatory adipokine of obesity and related metabolic disease. Little is known about its role in children. The aim of the current study was to determine the potential role of SPX in obese children and explore its relationships with obesity-related markers, insulin sensitivity and pancreatic β cell function. Method: We studied the levels of serum SPX in 40 obese and 32 normal weight pre-puberty children (mean age was 8.59 ± 1.82 and 8.15 ± 2.03 years in obesity and control groups respectively). We investigated the levels of body mass index, blood pressure, lipids, glucose, insulin, Homeostasis model assessment for insulin-resistant (HOMA-IR, HOMA for β-cell function [HOMA-β]), insulinogenic index and C-peptide index and analyzed their correlations with SPX levels. Results: SPX levels were significantly decreased in obese children compared to controls. Moreover, serum SPX levels were lower in IR obese subjects in contrast with the non-IR obese subjects. Serum SPX concentrations correlated negatively and significantly with triglycerides, systolic blood pressure, diastolic blood pressure, fasting insulin level, HOMA-IR, insulinogenic index, and HOMA-β levels in obese children. Conclusions: In summary, serum SPX levels significantly decreased in obese children and negatively correlated with insulin resistance and pancreatic β cell function indicators. Therefore, SPX may play a protective role in the process of glucose homeostasis and is closely related to β cell function in obese children.

Background Recently, copeptin and nesfatin-1 have been identified as interesting novel peptides playing a role in the pathogenesis of obesity-related insulin resistance in adults, respectively. However, the relationship between them has not been elucidated; and their circulating levels in obese children have not been adequately studied. Therefore, the current study aimed to investigate whether their levels are altered in Chinese obese children and to study the correlation of these two peptides with each other and with insulin resistance and other biochemical parameters. Methods A total of 120 children were enrolled in this study, including 80 obese children and 40 lean controls. Anthropometric parameters and clinical data of all subjects were collected and circulating TNF-α, adiponectin, leptin, copeptin and nesfatin-1 levels were detected using enzyme-linked immunosorbent assays. Results Serum copeptin and nesfatin-1 levels were significantly elevated in obese children and children with insulin resistance compared to control subjects. In addition, nesfatin-1 and copeptin levels were found to be significantly positively correlated with one another in pearson’s correlation and partial correlation. More importantly, multiple regression analysis used nesfatin-1or copeptin as the dependent parameters and significant correlation between nesfatin-1 and copeptin with each other and the associations between each of them with HOMA-IR were detected. Conclusion These findings are novel and shed light on the possible interplay role of these two molecules in obesity-related insulin resistance.
