Outcome of surgical treatment for displaced acetabular fractures: a prospective study

Abstract: ObjectiveThe aim of this study was to evaluate the functional outcome of surgically treated acetabular fractures.MethodsA prospective longitudinal study was undertaken in this hospital during the period from December 2010 to December 2014. A total number of 46 patients with the diagnosis of acetabular fracture were included in the study. The main cause of the acetabular injury was a road traffic accident. “…The quality of the fracture reduction appears to be the main determinant of the risk of late traumatic arthritis. Other complications include sciatic nerve injury, heterotopic ossification, and osteonecrosis of the femoral head [ 28 ]. In this study, two patients in the traditional method group who had radiologic displacement > 2 mm had posttraumatic arthritis of the affected hip within 18 months, and conversion to total hip replacement was performed.…”

“…Hence, in our study according to Matta's criteria good radiological outcome was achieved in 48.2% of the patients followed by excellent outcome in 29.6% and fair & poor outcome in 11.1% patients. A study by Lal SR 2018 23 showed excellent function outcome in 60.8% of the cases followed by good in 21.73%, fair in 8.69%, and poor in 8.69%. The postoperative complications were noted as osteoarthritis in 6.52% of the patients, heterotopic ossification in 2.17%, skin infections and avascular necrosis in 4.34% & nerve lesions in 2.17%.…”

“…Fractures of the acetabulum were treated nonoperatively till the middle of the 20 th century. After several progressions, acetabular surgery has become the accepted standard for the treatment of almost all displaced acetabular fractures [5] In posterior wall fracture, in case of adequate fragment size, one or two isolated inter-fragmentary lag screws is used to fix the reduced fragments, make sure to insert every screw extra-articularly [6] .…”
