Background and Purpose. Hip dislocation combined with acetabular fracture remains a challenging condition for orthopedic surgeons. In this study, we utilized a computer-assisted simulation and three-dimensional (3D) printing technology to treat patients with hip dislocation combined with acetabular fracture. We hypothesized that the 3D printing-assisted method would shorten the internal fixation time and surgical time. Methods. We retrospectively reviewed 16 patients diagnosed with traumatic posterior dislocation of hip combined with acetabular fractures and treated with plate fixation from September 2013 to August 2017. Patients were divided into two groups: (1) traditional method and (2) 3D printing groups. In the traditional method group, the plates were contoured during the surgery, whereas in the 3D printing group, the patient’s pelvic computed tomography image was transformed to the 3D medical image software for processing preoperatively. The fracture reduction was simulated by the computer. Thereafter, the 1:1 scale 3D printing model was used to design the surgical plan and contour patient-specific plates preoperatively. Results. The internal fixation time was significantly shorter in the 3D printing group than in the traditional method group (-33 min, P<0.05). The mean operative time was shorter than that in the traditional method group (-43 min). However, blood loss and postoperative radiograph results were similar between the groups. The complication rate was lower in the 3D printing group (2/7) than in the traditional method group (5/9). Interpretation. Computer-assisted simulation with 3D printing technology is a more efficient method for treating hip dislocation combined with acetabular fractures.

OBJECTIVE: To evaluate the radiological outcomes and post-operative complications of Acetabular Fractures treated with reconstruction plates. METHODOLOGY: It was a prospective descriptive study conducted at the Department of Orthopaedic Surgery, Dow University of Health Sciences Karachi from May 2013 to April 2016. An aggregate number of 27 patients who were diagnosed with Acetabular fracture were enrolled in the study. The primary cause of fracture was road traffic accident. All the patients were treated with reconstruction plates. Post-operative complications and radiological outcomes were evaluated. RESULTS: Total 27 patients with Acetabular fracture were treated. Excellent radiological outcome was achieved in 29.6% of the patients, good radiological outcome was achieved in 48.2%, fair& pair radiological outcome was achieved in 11.1% of the patients. Post-operative complications like sciatic nerve injury was developed in 2 patients, myositis ossificans was in One (01) patient and Four patients develop a vascular necrosis and Three (03) had infection. CONCLUSION: Acetabular fractures are now common fracture because of high velocity Road traffic accident. Acetabulum fractures should be fixed timely to achieve the anatomical reduction and favourable outcome with fewer complications.

Background: Acetabular fractures are a common clinical challenge. The posterior wall fractures represented 35% of all acetabular fractures and usually complexed. The curative treatment is surgical and depends on the fracture anatomy and surgeon preferences. Aim of the Work:The current study aimed to assess the short term results of posterior acetabular fractures management by open reduction and internal fixation.Patients and Methods: Thirty patients with displaced posterior wall acetabular fractures were included. They were treated by open reduction and internal fixation using neutralizing plates and screws. All patients were assessed by a preprepared trauma assessment sheet. Post-operative, active assisted and pain free passive range of motion exercises in all planes was advised. Functional outcome was evaluated using the Merle D'Aubinge and Postel score. Patients were followed up initially at 3 weeks intervals for first 2 months and thereafter at 6 weekly intervals for the next 6 months.Result: Patients were classified to two groups: Group [A] "70%" patients with isolated posterior wall fracture, and Group [B] "30%" of patients with posterior column plus posterior wall acetabular fracture. Both groups were comparable regarding age, gender, side and mode of trauma. The double plating fixation method was significantly increased in group B when compared to group A [77.8% vs. 0.0% respectively]. Group B was associated with significant increase of operative time than group A [164.44±13.33 vs. 128.57±27.80 minutes, respectively]. Blood loss significantly increased in group B than group A [1422.22±376.76 vs. 780.95±437.46 ml, respectively]. The excellent outcome was significantly associated with younger age, lower or absent complications, and anatomical [good quality] of direct postoperative radiological outcome. Conclusion:Most of our patients had an excellent and good outcome as a result to application of more strict selection criteria of patients and pattern.
