Introduction: Lipoma Arborescens is a rare pathology that mainly affects the knee. Occurrences in the elbow are even more uncommon and mainly involve the bicipitoradial bursa. Case's description: We describe the case of a 54-year-old patient known for rheumatoid arthritis, who consulted for chronic elbow pain associated with swelling and limited extension. Diagnosis: The diagnosis of a lipoma arborescens of the elbow involving the whole joint was made using magnetic resonance imaging and confirmed during arthroscopy. Interventions: After a failed nonoperative treatment consisting in intra-articular cortisone injections and physiotherapy, the patient underwent arthroscopic synovectomy and arthrolysis. Outcome: At 1-year follow-up, he reported no pain, satisfactory range of motion, and major improvements in clinical scores. Conclusion: This is the first illustrated case report about lipoma arborescens involving the whole elbow joint. Even though it is a rare disease, awareness of its presentation, imaging patterns, and treatment options is therefore important for clinicians, radiologists, and surgeons. In this case, arthroscopic treatment resulted in satisfactory and long-lasting pain relief and functional results. It may be considered as a safe and effective option in case of failed nonoperative measures.

Lipoma arborescens is a rare non-neoplastic condition that affects the synovial lining of joints and bursae accounting for less than 1% of all lipomatous lesions. Characterized by villous proliferation of the synovium, it is an uncommon cause of intra/periarticular mass presenting as a painless, slowly progressive longstanding swelling, and is sometimes accompanied by intermittent monoarticular effusions. We describe a rare case of bilateral lipoma arborescens in the bicipitoradial bursae in a young male referred for MRI evaluation of spontaneous bilateral elbow swelling. We chose to bring this case to light because of the rare simultaneous involvement of the bicipitoradial bursae bilaterally and the role of MR in providing a definite diagnosis, hence obviating the need for biopsy and avoiding consideration of other complex intra/periarticular masses.

ResumenEl lipoma arborescente es un tumor infrecuente, a pesar de ello se trata de un tumor que es más frecuente en la rodilla. Entre los diagnósticos diferenciales se encuentra la sinovitis villonodular, y otra serie de enfermedades articulares. El lipoma arborescente se suele presentar con clínica de derrame articular. En el caso que presentamos, la paciente acudía principalmente por tumoración en el antebrazo y síntomas de parestesias y disestesias en 2° y 3er dedos de la mano. Tras una adecuada exploración física y pruebas complementarias, fue diagnosticada de un Lipoma arborescente en la bursa bicipitoradial que generaba una compresión del N mediano a ese nivel. Tras la resección de la tumoración la paciente mejoró de su clínica.
