Objective:To demonstrate whether or not there is a correlation between the risk factors for gonarthrosis and the radiographic classification of Ahlbäck.Methods:We studied patients with primary gonarthrosis attended at the knee outpatient clinic of the General Hospital of Vila Penteado during their routine visit. We collected data on patient age (years), weight (kg), height (meters), body mass index (BMI = patient weight/height2), personal history of hypertension or diabetes mellitus (positive or negative), sedentarism (physical activity less than three times per week, 30 minutes per session), functional demand (how many blocks walked weekly), time of onset of symptoms (in years) and laterality or bilaterality. The data were correlated with the Ahlbäck classification applied to the radiographs performed at the time of the consultation.Results:A sample of 108 patients was studied. We did not find an association between the Ahlbäck classification and the patient's age, smoking, sedentary lifestyle, laterality, number of blocks walked per week, diabetes mellitus, and sex; however, a positive association was observed in hypertensive patients as well as a weak correlation with height and weight of the patient and moderate correlation with BMI.Conclusion:The Ahlbäck classification is unrelated to most of the risk factors for primary gonarthrosis. Level of evidence III, Case-control study.

Aim: The aim of this quantitative study was to assess falls and fear of falling in patients with knee osteoarthritis, and to determine their effects on daily activities. Design: A descriptive and prospective quantitative study. Methods: The study employed a descriptive and prospective design, examining 93 patients with knee osteoarthritis. We used the Lequesne Index (LI), the Falls Efficacy Scale (FES), and the Visual Analog Scale for Fear of Falling (VAS-FOF) to assess the effects of falls and fear of falling on daily activities. Results: The mean age of participants was 62.0 years old. In total, 78.5% of patients had experienced falls in the past, 53.4% of whom were admitted to hospital as a result. There was a positive significant correlation between total mean Lequesne Index and VAS-FOF scores, age, and BMI of participants (r = 0.310; r = 0.231; r = 0.283) (p < 0.05). A negative correlation was found between FES and VAS-FOF scores, total LI score, and age (p < 0.05). In addition, a negative significant correlation was found between age and BMI (r =-0.257) (p < 0.013). Conclusion: The results demonstrated that fear of falling affects the performance of daily activities. Knee osteoarthritis is an important risk factor for falling, and falls affect daily activities in those with knee osteoarthritis.

Resumen El propósito de este artículo es describir el manejo de un caso infrecuente de genu varo artrósico severo con defecto óseo tibial importante. Introducción: La deformidad en genu varo es el tipo de gonartrosis más frecuente, en estados avanzados conlleva a una importante pérdida ósea convirtiéndose en uno de los aspectos más desafiantes en el manejo de la artroplastia de rodilla. Caso clínico: masculino de 68 años, con disbasia de miembro inferior derecho, marcha con apoyo, a nivel de rodilla derecha se evidencia genu varo artrósico no reducible (Ahlbäck V), con rangos completos de flexo – extensión. Técnica quirúrgica: Realizamos un reemplazo articular total de rodilla derecha con vástago tibial de revisión más cuña metálica para defecto óseo de 12x24mm, durante el acto quirúrgico, se comprueba la estabilidad de implantes, el encarrilamiento rotuliano y rangos completos de movimiento. Resultados: valoración radiológica postquirúrgica satisfactoria, paciente evoluciona favorablemente, se estimula movilidad de rodilla, bipedestación y marcha de forma precoz. Discusión: la resección femoral como tibial debe ser menor de 8 mm si hay una deformidad severa o una subluxación tibial que indica inestabilidad en recurvatum. Con deficiencias óseas mayores de 10 mm, la tibia no debe cortarse al nivel del déficit, la resistencia del soporte óseo se reduce y el uso de un componente tibial más estrecho reduce el área de apoyo. Conclusiones: En nuestro caso se aconseja utilizar prótesis aumentadas con una cuña de metal reforzada por ser mecánicamente superiores al cemento o injertos en términos de osteointegración y estabilidad del implante.
