Combined anterior and posterior cruciate ligaments avulsion from the tibial side in adult patient: case report

Abstract: The authors describe a rare case of a 28-year-old male patient, victim of motorcycle crash, with direct impact on the right knee, who sustained a bicruciate ligament fracture avulsion from the tibial side, dislocated and with large dimensions, without associated ligamentary lesions; he has undergone surgical treatment – open reduction and internal fixation, of the avulsions, and the follow up was at least six months, presenting good outcome using the Tegner–Lysholm scale.

“…They are primarily documented in adults after high-energy impacts, such as two-wheeler vehicle accidents and falls from a height. [1][2][3][4][5] Hyperextension of the knee from such injuries places strain on both the ACL insertion in the tibial spine and the PCL insertion in the posterior aspect of the tibial plateau and is a common mechanism of injury for adult ACL and PCL avulsion fractures. 6 Similar to stand-alone cruciate ligament tibial avulsion fractures, BTAFs present with knee pain with weight bearing, antalgic gait, painful range of motion, and instability.…”

“…At present, the treatment of PCL injury mainly includes conservative treatment and reconstruction treatment [3]. In patients who need reconstruction therapy, how to select appropriate materials has been one of the hot topics [4].…”

“…Displaced fractures in younger active patients are typically treated with surgical stabilization, either by open or arthroscopic techniques, to minimize the risk of malunion or nonunion with subsequent stiffness, instability, and impingement [1][2][3][4]6,7 . Combined avulsion fractures of the ACL and PCL are extremely rare and have been rarely reported 8 . In this study, we present a case of a young healthy male patient who sustained displaced tibial avulsion fractures of both the ACL and PCL after falling off a bicycle.…”
