Meniere's disease or syndrome is one of the most common inner ear diseases. Meniere's disease is characterized by episodic vertigo, sensorineural hearing loss that fluctuates during episodes, tinnitus, and ear fullness. Ideal treatment should stop vertigo attacks, restore hearing, get rid of tinnitus and ear fullness. Treatment options are decided upon the remaining hearing, severity, and intensity of vertigo attacks. Meniere's disease is progressive on hearing levels of the patient; some of them develop profound hearing loss that also could affect the other ear. In order to plan a treatment scheme for patient, these conditions should be assessed. It has a destructive and progressive nature, so the first step of treatment should contain more conservative treatment options. If symptom control could not be obtained, destructive treatment options should be considered.

Currently, the pharmaceutical industry devotes great attention to drug degradation products because these compounds can offer risks to patients. A previous degradation study of betahistine (N-α-methyl-2-pyridylethylamine) conducted under different stress conditions detected three main impurities named A, B and C. Degradation products were analyzed by high-resolution mass spectrometry in electrospray source and time of flight analyzer (ESI-TOF) and nuclear magnetic resonance (NMR). Impurity mutagenicity was evaluated by Derek Nexus and Sarah Nexus softwares. Liquid chromatography hyphenate with tandem mass spectrometry (LC-MS/MS) analysis of the betahistine forced degradation sample indicated the presence of a new impurity, which was named impurity C1. 2D NMR experiments allowed the complete structural characterization of the new entity. The active pharmaceutical ingredient and degradation impurities were classified as inactive in the in silico mutagenic studies. Systematic investigation of a forced degradation sample led to the characterization of a new betahistine impurity. The in silico mutagenicity study of the betahistine degradation impurities may be useful in the risk assessment of the drug products.

<p>Vertigo is a condition associated with a spectrum of symptoms and ~30% of general population experience vertigo in their life time. In spite of being of high clinical importance, the management of vertigo is quite challenging. Though the literature supports the availability of various therapeutic interventions used in vertigo treatment, their effectiveness depends on accurate diagnosis, appropriate use of intervention, and physician’s awareness of the overlap between vestibular, autonomic, and psychological aspects of vestibular pathology. Unfortunately, several drugs act as tranquilizers and impede the process of vestibular compensation. Betahistine, a histamine analogue, is one of the most commonly used anti-vertigo drugs worldwide and has been supported by many clinical trials. There have been several oral communications in international conferences on the efficacy of using betahistine in several clinical vertiginous syndromes. The current review assesses the use of betahistine 48 mg twice daily for three months as an efficient and well-tolerated treatment for vertigo. Additionally, it highlights the low incidence of side effects even at high doses of betahistine and suggests that it may be considered as the first-line of treatment for vestibular dysfunction.</p>

Abstract: Oral betahistine was significantly effective in preventing/reversing hearing deterioration in patients with Ménière's disease. Age, the hearing level on admission, and the disease duration were independent predictors of hearing status after treatment.
