Transmitted drug resistance in patients with acute/recent HIV infection in Brazil

Abstract: The high prevalence of nonnucleoside reverse transcriptase inhibitors resistance indicates that efavirenz-based regimen without prior resistance testing is not ideal for acutely/recently HIV-infected individuals in our setting. In this context, the recent proposal of including integrase inhibitors as a first line regimen in Brazil could be an advantage for the treatment of newly HIV infected individuals. However, it also poses a new challenge, since integrase resistance test is not routinely performed for anti… Show more

“…Azidothymidine (AZT), lamivudine (3TC), and tenofovir (TDF), as rst-line NRTI drugs in China, have meager rates of transmission resistance (0.3%, 0.15%, and 0%, respectively). However, unlike the ndings reported in other studies, [4,5,[36][37][38][39][40] NRTI resistance showed the most signi cant increase (from 0% to 5.17%) from 2009 to 2017 in this study. Although the prevalence of TDR to PIs (0.44%) was signi cantly lower than the prevalence of TDR to NNRTIs/NRTIs, the I54M mutation caused universal resistance to PI drugs.…”

“…A cohort of Brazilian individuals presenting recent HIV-1 infection has been followed-up since August 2013 at the Instituto Nacional de Infectologia Evandro Chagas (INI-FIOCRUZ). These individuals initiated cART immediately after diagnosis and have been described elsewhere (Ferreira et al, 2017). For the present study, we selected only individuals, who started cART during the acute phase (Fiebig II–V) (Fiebig et al, 2003) of HIV-1 infection ( n = 10).…”

“…DRMs monitoring is an important tool for surveillance of drug-resistant variants, and genotyping test is very useful to define salvage therapy. 29 DRMs that may jeopardize the success of HIV treatment must be carefully tracked, as ARV drug therapy is still the only feasible treatment option to suppress the virus. 16,23,30 According to the 2018 report, the plan of action for the prevention and control of HIV and sexually transmitted infections 2016-2021 by WHO, surveillance of drug resistance continues to be fundamental to enable an effective drug monitoring of the currently available drugs and also of putative future drugs, and also to inform of policy changes in public health treatment.…”
