Squatting is a common daily activity and fundamental exercise in resistance training and closed kinetic chain programs. The aim of this study was to investigate the effects of an experimentally induced weakness of the gluteal muscles on joint kinematics, reactions forces (JRFs), and dynamic balance performance during deep bilateral squats in healthy young adults. Ten healthy adults received sequential blocks of (1) branch of the superior gluteal nerve to the tensor fasciae latae (SGNtfl) muscle, (2) superior gluteal nerve (SGN), and (3) inferior gluteal nerve (IGN) on the dominant right leg. At the control condition and following each block, the participants were instructed to perform deep bilateral squats standing on two force plates. Hip, knee, ankle, and pelvis kinematics did not differ significantly following iatrogenic weakness of gluteal muscles. The most important finding was the significant differences in JRFs following SGN and IGN block, with the affected hip, patellofemoral, and ankle joint demonstrating lower JRFs, whereas the contralateral joints demonstrated significantly higher JRFs, especially the patellofemoral joint which demonstrated an average maximum difference of 1.43 x body weight compared with the control condition. When performing a deep bilateral leg squat under SGN and IGN block, the subjects demonstrated an increased center of pressure (CoP) range and standard deviation (SD) in mediolateral compared with the control condition. These results imply that squat performance changes significantly following weakness of gluteal muscles and should be considered when assessing and training athletes or patients with these injuries.

BACKGROUND: Peripheral nerve stimulation (PNS) is an effective alternative for the management of neuropathic peripheral chronic pain, but the high incidence of adverse events such as lead and battery erosion, migration, lead fracture, disconnection, and infection have limited the widespread use of PNS. Neuromodulation technology that does not include implantable pulse generators (IPGs) but a 4- or 8-contact electrode array with embedded electronics and a small, externally worn rechargeable transmitter reduces the complications related to the implant of an IPG. PNS has traditionally been performed with a tonic stimulation protocol. This case series describes a wireless PNS device at subthreshold frequencies for the treatment of neuropathic pain of peripheral nerve origins. CASE REPORT: No adverse events were reported, and no complications were encountered during implantation. All patients reported more than 50% pain relief during the one-week trial period, sustained pain relief with various placements and number of electrodes, and an important improvement in quality of life and sleep. Mean VAS scores decreased 78% at one month (n = 11) and remained stable at 6 months with 91% reduction (n = 5) and 76% reduction (n = 1) at 12 months. Mean PGIC at 6 months was 7 of 7. CONCLUSION: Percutaneous placement of an externally powered neurostimulation device adjacent to the affected peripheral nerve(s) is an effective, minimally invasive, and reversible method of pain control in patients with neuropathic pain. PNS using subthreshold frequencies effectively controls neuropathic pain from multiple peripheral nerve targets. Since the widespread introduction of ultrasound for assisting techniques used in anesthetic theatres, peripheral nerve blocks of the trunk have been used more frequently to produce analgesia and anesthesia for surgeries involving the thorax, abdomen, and lower extremities. The introduction of real-time ultrasound has allowed the peripheral nerves, planes, and plexuses of the trunk to be located more accurately and has improved the success rate of truncal blocks The widespread application of ultrasound in clinical anesthesiology has led to an increase in the number of practitioners performing peripheral nerve blocks of the trunk to produce analgesia and anesthesia for surgeries involving the thorax, abdomen, and lower extremities. The use of real-time ultrasound has allowed the peripheral nerves, planes, and plexuses of the trunk to be located more accurately and has improved the success rate of blocks. Compared to central neuraxial techniques, many types of truncal blocks are relatively easy to perform and have low side effect profiles. As a result, truncal blocks have been introduced to provide perioperative analgesia for surgeries involving the thorax and abdomen. The most frequently used truncal block techniques include the paravertebral block, intercostal block, pectoralis and serratus anterior block, rectus sheath block, transversus abdominis plane block, and ilioinguinal and iliohypogastric nerve blocks. Recently introduced techniques also include the quadratus lumborum and erector spinae plane blocks. To apply each block correctly and reduce the likelihood of related side effects and complications, the practitioner must have a thorough understanding of the anatomical region, optimal block positioning, and device selection and management.
