Background: Supreme laryngeal mask airway (SLMA) and I-gel airway devices are second generation supraglottic airway devices (SAD) and are good alternatives to intubation during surgeries. The study was conducted with the objective to compare two supraglottic airway devices for ease of insertion, number of attempts of insertion, hemodynamic changes, incidence of adverse effects like regurgitation, lip and dental trauma and post-operative sore throat, dysphagia or hoarseness. Methods: This study was conducted at Topiwala National Medical College and BYL Nair hospital, Mumbai. 80 patients of ASA class 1 and 2 with Mallampati grading 1 and 2, between age group of 18-60 years and with BMI <28kg/m 2 were selected for the study. After induction of anesthesia for all the patients, one of SAD (SLMA or I-gel) was inserted randomly and accordingly they are divided into two groups consisting of 40 in each. Insertion parameters, hemodynamic and respiratory parameters were noted. Patients were also observed for any complications for 1 hour and 24 hours postoperatively. Results: Both groups were compared in terms of demographic characteristics, insertion parameters, hemodynamic and respiratory parameters and found no statistically significant differences between them (p>0.05). Postoperatively no significant complications were observed in terms of dental injury, laryngospasm. Complication like sore throat after 1 hour and after 24 hours was comparatively more in I-gel group but difference was not significant at 1 hr (p>0.05). Dysphagia was reported more in SLMA group (8 cases) than I-gel group (1 case) at one hour and the difference was statistically significant (p=0.013). It has integral bite block and a tab for adhesive fixation of the device. Fixation tab is rectangular structure moulded on to the manifold at right angles and projects over the patient's upper lip. INTRODUCTIONThe LMA Supreme™, i-gel ® and LMA ProSeal™ are second-generation supraglottic airway devices. We tested the hypothesis that these devices differ in performance when used for spontaneous ventilation during anaesthesia.METHODS 150 patients who underwent general anaesthesia for elective surgery were randomly allocated into three groups. Data was collected on oropharyngeal leak pressures, ease and duration of device insertion, ease of gastric tube insertion, and airway safety. RESULTSLeak pressure, our primary outcome measure, was found to be higher for the i-gel than the Supreme and ProSeal (mean ± standard error of the mean: 27.31 ± 0.92 cmH 2 O, 23.60 ± 0.70 cmH 2 O and 24.44 ± 0.70 cmH 2 O, respectively; p = 0.003). Devices were inserted on the first attempt for 90%, 82% and 72% of patients in the i-gel, Supreme and ProSeal groups, respectively (p = 0.105); mean device placement times were 23.58 seconds, 25.10 seconds and 26.34 seconds, respectively (p = 0.477). Gastric tubes were inserted on the first attempt in 100% of patients in the Supreme group, and 94% of patients in the i-gel and ProSeal groups (p = 0.100). There was blood staining on removal in 9 (18%) patients in each of the Supreme and ProSeal groups, with none in the i-gel group (p = 0.007). The incidence of postoperative sore throat, dysphagia and hoarseness was lowest for the i-gel. CONCLUSIONThe three devices were comparable in terms of ease and duration of placement, but the i-gel had higher initial oropharyngeal leak pressure and lower airway morbidity compared with the ProSeal and Supreme.

Objectives:To compare the efficacy and performance of the pediatric Ambu AuraOnce (Ambu AO) mask (Ambu, Copenhagen, Denmark) and i-gel mask (Intersurgical Ltd., Wokingham, United Kingdom).Methods:From May 2015 to September 2016, 112 patients, 0-14 years old, underwent elective surgery at a tertiary university hospital (Riyadh, Saudi Arabia). They were randomly assigned to the Ambu AO or i-gel group. Three groups underwent a subgroup analysis: ≤5 kg (group 1), 5.1–10.0 kg (group 2), and >10 kg (group 3).Results:The oropharyngeal leak pressure was significantly higher for the i-gel (25.4±4.1 cm H2O) than for the Ambu AO (22.5±3.9 cm H2O, p<0.001). The Ambu AO had a slightly higher ease of insertion compared to the i-gel (100% versus 94%, p=0.08) and required less manipulation (2% versus 11%, p=0.07).The Ambu AO and i-gel showed non-significant differences in performance between weight groups. There were statistically significant differences for higher leak pressure in group 2 (p=0.01) and group 3 (p=0.002) in favor of the i-gel, and for less manipulation in the Ambu AO in group 1 (p=0.04). Fiberoptic viewing was superior in group 2 for the i-gel (p=0.03) and in group 3 for the Ambu AO (p=0.02).Conclusion:Both devices demonstrated equally good performance with low morbidity. The Ambu AO had a statistical tendency towards easier insertion and less manipulation. Confirming this finding will require large scale trials.

Abstract: Both devices had good performance with low postoperative complications and without occurrence of regurgitation. The i-gel provided a shorter insertion time and a better fiberoptic view than the Laryngeal Mask Airway Classic.
