Dermoscopy – a simple and rapid in vivo diagnostic technique for tinea incognito

Abstract: Tinea incognito resulting from corticosteroid abuse is becoming very common in the tropics. Its diagnosis is tricky owing to its confusing morphology, as well as practical and technical issues associated with mycological tests. Dermoscopy has now evolved as a novel diagnostic tool for diagnosing tinea incognito in such challenging situations, since the typical hair changes such as Morse-code hairs, deformable hairs, translucent hairs, comma and cork screw hairs, and perifollicular scaling may be seen despite s… Show more

“…Recently, dermoscopy was used for tinea incognita resulting from corticosteroid abuse. 12 Morse-code hairs, deformable and 1c translucent, comma and cork screw hairs, and perifollicular scaling were significant findings 13 in this Indian and Iranian study. The problem remains, however, the impossible species identification which is necessary in chronic recalcitrant dermatophytosis.…”

“… 80 , 81 These changes may be seen despite the use of topical corticosteroids or calcineurin inhibitors. 82 Reflectance confocal microscopy is another useful diagnostic tool. 2 , 83 With reflectance confocal microscopy, branching fungal hyphae can be detected over an erythematous annular scaly patch in individuals with tinea corporis.…”

“…In cases affecting non‐glabrous skin, comma, corkscrew, and Morse code‐like hairs can be observed. 12 , 15 , 16 In addition, concentric erythematous areas separated by scales, follicular micropustules, and black spots surrounded by a white‐yellowish halo can be detected. 12 , 17 Scaly, broken, translucent, and deformable hairs have also been reported.…”

“… 12 , 17 Scaly, broken, translucent, and deformable hairs have also been reported. 12 , 15 , 16 Such dermoscopic features in a patient presenting with long‐standing dermatitis and history of steroid or calcineurin inhibitor use should raise suspicion of tinea incognito.…”
