“…The pattern we observed for progressively increasing levels of global disability or dependency (as per the Schwab and England scale but not the UPDRS-ADL scale) with worsening of cognitive impairment across the 3 groups supports our initial hypothesis and is consistent with the previous findings of a linear association between factors such as overall disease severity of PD and level of cognitive impairment. 29 Our own group has recently shown that in PD without dementia, 56% of the variance in disability could be accounted for by higher levels of apathy and depression, longer duration of disease, and more impaired motor function. 6 As PD progresses from the stage with intact cognition to PD-MCI and finally PDD, apathy and motor impairment also progress and disease duration lengthens, and these factors are all likely contributing to gradually increasing levels of disability, which is a multifactorial concept.…”
