Objective To evaluate the incidence and clinical relevance of silent nontarget occlusion (NTO) of superficial and deep veins occurring after ultrasound-guided foam sclerotherapy (UGFS) that can be detected by serial duplex ultrasound scan (DUS). Methods This retrospective analysis evaluated the medical records of patients treated with UGFS at a private clinic in Moscow, Russia from 2015 to 2017. All patients underwent serial DUS at 1 to 2 weeks and 1, 3, 6, and 12 months after UGFS. Results During the observation period, 268 patients were treated with UGFS, using physician compounded polidocanol foam 257 lower limbs of 196 patients (73%) with varicose veins who underwent DUS at 1 to 2 weeks after the last session of UGFS (inclusion time-point): 139 women and 57 men (mean age: 44.2 ± 12.2 years) with the following CEAP clinical class distribution: C2, 74.0%; C3, 20.0%; C4, 4.5%; and C5, 1.5%. NTO at the inclusion time-point occurred in 60 limbs (23.3%) of 57 patients (28.5%) and was symptomatic only in three limbs (1.2%). Most occlusions were localized in the untreated great saphenous vein trunk ( n = 28) and the calf muscle veins ( n = 23). Overall, 72%, 44%, 29%, and 10% of all limbs and 77%, 45%, 28%, and 12% of limbs with NTO were followed up by DUS at 1, 3, 6, and 12 months, respectively. There were no cases of thrombus progression or symptomatic venous thromboembolism (VTE). At six months, no deep vein occlusions persisted. Conclusions The frequency of nontarget vein occlusion after UGFS revealed by serial DUS may be as high as 23.3%. These occlusions tend to resolve within six months and are not associated with symptomatic VTE.
Цель исследования-оценить влияние электрической стимуляции мышц или электромышечной стимуляции (ЭМС) голени на динамику резидуальной венозной обструкции (РВО) у пациентов с посттромботической болезнью (ПТБ) после завершения стандартного курса антикоагулянтной терапии. Материал и методы. Проведено проспективное сравнительное клиническое исследование с участием 60 пациентов (38 мужчин и 22 женщины) в возрасте от 40 до 86 лет (средний возраст 58,5 года), перенесших первый эпизод клинически неспровоцированного бедренно-подколенного венозного тромбоза, завершивших стандартный курс антикоагулянтной терапии, имеющих ультразвуковые признаки РВО, а также 5 баллов и более по шкале Villalta. Участники были разделены на две группы: основную и контрольную, по 30 человек в каждой. В обеих группах назначали ношение компрессионного гольфа (23-32 мм рт.ст.), курсовой прием флеботропных препаратов и дозированную ходьбу (не менее 5 тыс. шагов в сутки). В основной группе также использовали ежедневную ЭМС голени аппаратом. Срок наблюдения составил 12 мес с ежемесячной клинической и ультразвуковой оценкой. Оценивали частоту рецидива венозных тромбоэмболических осложнений, уменьшение степени РВО, уменьшение выраженности признаков ПТБ. Результаты. РВО была представлена стенозом общей бедренной вены в среднем на 48% у 12 пациентов, стенозом поверхностной бедренной вены в среднем на 53% у 16 больных и стенозом подколенной вены в среднем на 55%-у всех испытуемых. За период наблюдения эпизодов легочной эмболии не наблюдали. Рецидив венозного тромбоза был зарегистрирован у 7 пациентов контрольной группы и ни у одного из больных, применявших ЭМС: 23,3% против 0% (р=0,011). Уменьшение степени РВО было зарегистрировано в обеих группах. Разницу между группами наблюдали лишь в динамике РВО в подколенной вене: 60,8-55,1-28,8% в основной группе и 50,9-30,1-27,3% в контрольной группе (р<0,0001). Тяжесть ПТБ уменьшилась в обеих группах. Более выраженное снижение показателей наблюдали на фоне применения ЭМС (р<0,0001). Заключение. При наличии РВО после завершения стандартного курса антикоагулянтной терапии процесс реканализации пораженных вен не прекращается, а продолжается активным образом. Включение ЭМС в состав комплексной терапии ПТБ позволяет ускорить процесс реканализации и снизить частоту возникновения рецидива венозного тромбоза. In the modern conditions of unfavourable environmental situation in most cities, the problem of reproductive disorders (infertility and miscarriage) is becoming increasingly important. Oft en this can occur due to diseases of the endocrine system, in particular, due to autoimmune damage to the thyroid gland. Th e most modern method of combating infertility today is the programme of assisted reproductive technologies (ART), although its eff ectiveness may decrease due to the presence of autoimmune thyroiditis (AIT). Research data on the eff ectiveness of ART in females with AIT are extremely contradictory. However, most evidence suggests that superovulation induction in ART programmes elevates the risk of thyroid dysfunction in women with AIT. Patients with AIT are always at risk of hypothyroidism development in early pregnancy. Th erewith only L-thyroxine treatment is presently provided for women with infertility or miscarriage in combination with AIT who decide to use the in-vitro fertilisation (IVF) programme, which does not always show high effi cacy. Further studies are required to assess the role of AIT in reproductive disorders, as well as to develop alternative methods for AIT correction, including the condition of euthyroidism.
