Cervicogenic headache has a complex and poorly understood pathophysiology. Symptoms are typical and involve the C2 and C3 nerve roots. There are no specific recommendations for physical therapy, although occipital nerve block is a known pharmacological treatment. Some evidence suggests that hip rotation correction could aid in reducing cervical pain symptoms. The objective of this work is to evaluated the role of postural physiotherapy for hip rotation correction using the Maitland technique in patients with cervicogenic headache who underwent an occipital nerve block. In this retrospective, observational and uncontrolled study, patients were evaluated from January 2017 to February 2018. After diagnosis of cervicogenic headache they underwent anesthetic block, hip radiography with lower limb scanometry and cervical tomography, after which they were referred to physical therapy. The analog pain scale was used for evaluation at the time of diagnosis and after the proposed physiotherapy. Patients submitted to an occipital nerve block were divided into three groups: Group I (n = 15, physical therapy with Maitland technique) had an analog pain score of 1.6 after the physical therapy, Group II (n = 11, conventional physiotherapy) had an analog pain score of 3.7 after the physical therapy and Group III (n = 13, occipital nerve block only) had an analog pain score of 5.2 after the occipital nerve block. The results suggest that an occipital nerve block in combination with the Maitland technique is superior in patients with cervicogenic headache compared to nerve block only or blockade combined with non-specific physical therapy (p=0,013).
Cervicogenic headache is a clinical condition that has a direct impact on quality of life and labor productivity and its underdiagnosis and lack of consensus on the appropriate treatment aggravates even more the condition. This work aims to systematize the protocol that has the highest success rate so far, based on the study carried out in Pindamonhangaba, São Paulo, Brazil. The results demonstrated with the occipital block and Maitland physiotherapy were exceptional and its methodology will be carried out as performed during the study. Conclusion: Regardless of the substance used for the blockade, its combination with Maitland's physiotherapy was shown to be favorable and its description can help other professionals and also enable reproduction in other studies.
Objetivo: Descrever os principais mecanismos da autofagia estimulada por atividade física e sua relação com a redução na incidência de câncer. Material e métodos: Foram realizadas coletas de dados através da análise da literatura científica disponível nas bases de dados da MEDLINE, ScienceDirect e Wiley, de maneira independente e manual. Foram utilizados como descritores de busca os termos: “cancer”, “autophagy” e “physical activity”, e o período de tempo para as publicações compreendeu os anos de 1990 a 2020. O critério de inclusão foi: “trabalhos publicados dentro do escopo do estudo, publicados no intervalo de tempo citado”. E como critério de exclusão: “trabalhos não relacionados ao tema de estudo e com relatos já ultrapassados de acordo com a literatura atual”. Resultados: A atividade física influência positivamente diversos sistemas, levando a alterações agudas e crônicas. Os benefícios em patologias metabólicas, imunes, cancerígenas, cardiovasculares, psiquiátricas e neurológicas são indiscutíveis e os mecanismos estão sendo elucidados. Autofagia medeia a digestão e reciclagem de partes obsoletas da célula durante a inanição e participa de uma variedade de processos fisiológicos de remoção residual. A autofagia é capaz de eliminar as microorganimos, agregados proteicos tóxicos, genotóxicos, desempenhando papéis durante a infecção, envelhecimento e patogênese de doenças. Conclusão: A partir da análise dos mecanismos envolvidos na autofagia induzida por atividade física ficaram postulados três pontos principais: 1) Via mTORC1 inativada por inanição, 2) Regulação por espécies reativas de oxigênio e hipóxia, 3) Regulação da autofagia por vias alternativas à mTORC1.
A pandemia desencadeada pela COVID-19 vem assolando o mundo desde dezembro de 2019 pois sem nenhum tratamento específico conhecido houve o óbito quase 3 milhões de indivíduos no mundo. Sua transmissão ocorre de pessoa para pessoa e seus principais sintomas são febre, tosse seca e fadiga. O objetivo desse estudo foi analisar o prognóstico de pacientes com SARS-CoV-2, internados em uma UTI do Oeste do PR, através dos parâmetros hematológicos e sintomatológicos e assim auxiliar no estabelecimento de protocolos e condutas. Dos dados obtidos, verificou-se maior número de óbitos entre pacientes idosos (p=0,01) e o perfil laboratorial de pior prognóstico foi aquele com hematocritopenia, leucocitose, linfocitopenia e neutrocitose, além da maior associação à sintomas respiratórios. Também se constatou a não associação do sexo com as chances de óbito (p=0,13) ou tempo de internação (p=0,36). Sendo assim, novos estudos podem utilizar desses marcadores laboratoriais e sintomatológicos, assim como da idade, para auxiliar na rotina médica frente a pandemia.
Objetivo: Contextualizar a cefaleia cervicogênica descrevendo sua etiologia, fisiopatologia, quadro clínico, critérios diagnósticos e tratamentos, para nortear a sociedade médica diante da patologia que está em crescente aumento de sua incidência. Métodos: O presente estudo realizou buscas nas bases de dados PubMed, Nature, Scielo e Wiley, utilizando os descritores cefaleia cervicogênica, e foram analisados trabalhos publicados entre os anos de 1980 e 2022, em todos os idiomas, além das respectivas traduções em inglês. Como fator de inclusão foi considerado: “trabalhos publicados dentro do escopo do estudo dentro do intervalo de tempo citado e relacionados a cefaleia cervicogênica e como fator de exclusão foi considerado: “trabalhos não relacionados ao tema de estudo e com relatos já ultrapassados de acordo com a literatura atual”. Resultados: Foram encontrados 1.319 artigos, após leitura e análise dos artigos foram selecionados 27 artigos, e de acordo com a relevância no assunto fazem parte do escopo do trabalho. Em relação a cefaleia cervicogênica é possível classifica-la como uma cefaleia secundária, atribuída a transtornos cervicais, com sintomatologia heterogênea, geralmente apresentando-se como uma cefaleia unilateral, não latenjante e não excruciante, podendo ser desencadeada por pontos gatilhos em região cervical podendo ainda se apresentar com pródomos autonômicos. Sua etiologia e fisiopatologia tem ligação direta com transtornos cervicais e irritação das fibras aferentes de C1-C2-C3, além da convergência para o núcleo trigêmeo-cervical aumentando a variabilidade de sintomas. Seu diagnóstico é baseado em critérios diagnósticos e existem uma grande variedade de tratamentos com eficácia limitada. Conclusão: A cefaleia cervicogênica pode se apresentar de formas heterogêneas dificultado seu diagnóstico e sendo subdiagnosticada e tratada erroneamente em até 50% dos casos, seu aumento em decorrência da pandemia alerta para a melhoria no diagnóstico e tratamento da cefaleia cervicogênica e os distúrbios osteomusculares associados.
