Foi realizado um estudo qualitativo em Equipes de Saúde Bucal (ESB) do Programa de Saúde da Família (PSF), no Município de Resende, no Estado do Rio de Janeiro, visando avaliar a integralidade da assistência e a continuidade do cuidado à saúde para o câncer bucal. Em função da importância do câncer bucal e das condições das ESB do PSF em poder fazer o diagnóstico precoce, este trabalho se propõe a estudar, no município, as condições de diagnóstico e continuidade do cuidado para o câncer bucal. O objetivo deste estudo é analisar as condições técnicas e operacionais dos cirurgiões dentistas das ESB para identificação, diagnóstico precoce e encaminhamento de pacientes cadastrados nas suas áreas de atuação, com suspeita de lesões orais consideradas pré-cancerosas. Para obtenção dos dados primários, foram utilizadas técnicas de entrevistas, com roteiros semiestruturados Assim, quantitativamente tivemos: 11 dentistas do PSF no município de Resende e os profissionais do CEO. A entrevista semiestruturada, parte do trabalho de campo, foi elaborada contendo perguntas fechadas e abertas. Após levantamento e consolidação dos dados qualitativos e quantitativos, podemos constatar que o município conta com uma rede de saúde articulada, em que os pacientes com casos de lesões suspeitas são encaminhados para o centro de referência para continuidade do cuidado. Entretanto, podemos observar que a cobertura de 45,94% da população de Resende pelas Equipes de Saúde Bucal, correspondendo a 59.722 habitantes, se constitui num fator limitante para a detecção precoce de lesões suspeitas de câncer bucal. Dessa forma, torna-se importante a ampliação da oferta de serviços de saúde bucal, objetivando não só aumentar a resolubilidade, evitando transtornos adicionais aos pacientes, como dar mais agilidade ao diagnóstico, contribuindo para a qualidade na sobrevida dos pacientes com diagnóstico positivo de câncer oral.
O conhecimento da anatomia interna dos dentes humanos é essencial para se conseguir o sucesso na terapia endodôntica. O quarto canal conhecido como mesiopalatino em molares superiores, sempre foi motivo de discussão e polêmica. Foram analisados 50 primeiros molares superiores humanos, em que foi realizado o acesso convencional. Nos dentes em que não se constatou a presença do quarto canal, foi confeccionado um desgaste, na sua possível embocadura, e esses elementos foram então, novamente avaliados pelos Cirurgiões Dentistas. Após essa manobra, os dentes em que o quarto canal (mesiopalatino), mais uma vez, não foi observado por nenhum dos Cirurgiões Dentistas, tiveram a raiz mesiovestibular lixada, até a sua camada mais interna, com o objetivo de se constatar a presença ou não do quarto conduto. Com a tinta nanquim azul turquesa foi pintado o canal mesiovestibular e com o nanquim vermelho o canal mesiopalatino. O quarto canal mesiopalatino foi encontrado em 27 primeiros molares superiores (54%) com acesso convencional, com desgaste na região de sua embocadura em 18 dentes (36%), dos 5 dentes restantes que tiveram suas raízes lixadas, 4 não tinham o canal mesiopalatino (8%), e apenas 1 desses dentes possuía o quarto canal (2%). Pôde-se concluir que ocorreu uma elevada incidência da presença anatômica do quarto canal nas raízes mesiovestibulares dos primeiros molares superiores e que o desgaste na região de sua embocadura é essencial para se achar esse conduto e para que se tenha sucesso no tratamento endodôntico do primeiro molar superior permanente.

O presente trabalho se propõe a fazer um paralelo entre a construção histórica do Sistema Único de Saúde brasileiro e o processo de mecanização e modernização do trabalho, especialmente no Brasil, para que possamos, a partir dessas reconstruções históricas, entendermos como a Odontologia do Trabalho se insere como importante ferramenta e instrumento para a qualidade de vida deste “novo” trabalhador, exposto a novas patologias e a novas formas e relações de trabalho moderno. Não temos a pretensão, neste estudo, de detalharmos, passo a passo, a modernização do Brasil com interface nas respectivas conjunturas políticas, mas sim, de atentarmos para como o Brasil instalou e transformou seu parque industrial e, como neste cenário, a saúde do trabalhador necessita também avançar, e ser entendida de maneira mais ampla, do que somente a ausência de patologias. Nesse espectro, a Odontologia do Trabalho se coloca como forte ferramenta para concretização do direito constitucional ligado, principalmente, a integralidade da assistência.
