Inherited prion diseases are characterized by mutations in the PRNP gene encoding the prion protein (PrP). We report a novel missense mutation in the PRNP gene (resulting in a G114V mutation in PrP) in members of a Uruguayan family with clinical and histopathologic features of prion disease. Affected individuals were characterized by an early age at onset, initial neuropsychiatric symptoms, late dementia with prominent pyramidal and extrapyramidal symptoms, and long disease duration.
ARTICLE Cerebrospinal fluid examination may be useful in diagnosing neurosyphilis in asymptomatic HIV+ patients with syphilisEl examen del líquido céfalo raquídeo puede ser útil en el diagnóstico de neurosífilis en pacientes VIH + asintomáticos con sífilis Whether lumbar puncture should be indicated for diagnosing neurosyphilis in patients who are coinfected with the human immunodeficiency virus (HIV) and Treponema pallidum (T. pallidum) but do not have any symptoms or neurological signs is a matter for discussion 1,2 . The European guidelines recommend lumbar puncture in cases of symptomatic patients (neurological, ophthalmological or otological symptoms) and asymptomatic HIV-positive patients with Venereal Disease Research Laboratory (VDRL) test results greater than or equal to 1/32 in serum and CD4 less than or equal to 350. These guidelines suggest that lumbar puncture should not be compulsory but that it could be indicated in cases of late latent syphilis or syphilis of undetermined duration 3 . In 2002, the Centers for Disease Control and Prevention (CDC) recommended lumbar puncture for patients with late latent syphilis or syphilis of undetermined duration. This recommendation was later revised to include reactive VDRL ≥ 1/32 in serum and CD4 cell counts ≤ 350 cells/ml. Since 2010, CDC has not been recommending lumbar puncture for asymptomatic patients. AbstrACtLumbar puncture in neurologically asymptomatic HIV+ patients is still under debate. There are different criteria for detecting neurosyphilis through cerebrospinal fluid (CSF), especially in cases that are negative through the Venereal Disease Research Laboratory (VDRL), regarding cellularity and protein content. However, a diagnosis of neurosyphilis can still exist despite negative VDRL. Treponema pallidum hemagglutination assay (TPHA) titers and application of the TPHA index in albumin and IgG improve the sensitivity, with a high degree of specificity. Thirty-two patients were selected for this study. VDRL was positive in five of them. The number of diagnoses reached 14 when the other techniques were added. It was not determined whether cellularity and increased protein levels were auxiliary tools in the diagnosis. INTRODUCCION: Listeria monocytogenes tiene una especial predilección por infectar el sistema nervioso central y sus cubiertas meningeas. Afecta a pacientes que se encuentran en edades extremas de la vida, pacientes con deficiencia en su inmunidad celular y adultos sanos. La forma mas común de manifestarse es la meningitis aguda, aunque puede expresarse como cerebritis, encefalitis de tronco (romboencefalitis), y excepcionalmente mielitis. CASUISTICA: Se presentan y comentan seis casos clinicos de neurolisteriosis, cinco en adultos sanos, con sus hallazgos imagenológicos y licuorales. RESULTADOS: Tres de los pacientes se presentaron como meningitis aguda, uno como meningoencefalitis, otro como cerebritis y el restante como romboencefalitis. Se destaca el carácter turbio o ligeramente turbio del líquido cefalo-raquideo (LCR), la glucorraquia normal detectada en tres de los casos y el diagnostico realizado en cinco de los casos por cultivo del LCR. Se comenta la resonancia magnética singular del caso de la romboencefalitis con microabscesos en tronco. Todos los pacientes tuvieron evolución satisfactoria con tratamiento antibiotico. CONCLUSIÓN: La neurolisteriosis debe ser un diagnostico a tener en cuenta no solo en pacientes inmunocomprometidos o en edades extremas de la vida. Debe también tenerse en cuenta en pacientes adultos jóvenes sanos procedentes de regiones donde las condiciones sanitarias son precarias y no existe un adecuado control en la elaboración de alimentos.
La nocardiosis del sistema nervioso central (SNC) es una enfermedad poco común, cuya frecuencia ha aumentado con el crecimiento de los tratamientos inmunosupresores. El hombre se infecta por inhalación, inoculación traumática cutánea directa y tras una infección periodontal ingiriendo alimentos contaminados. La localización pulmonar es la más frecuente, siendo la fuente inicial de diseminación hemática, con predominio en piel, tejido celular subcutáneo y SNC. Es conocida la predilección de Nocardia por el SNC. Estos abscesos, asociados a altos índices de mortalidad, especialmente en pacientes inmunocomprometidos, siguen constituyendo un desafío diagnóstico y terapéutico, permaneciendo no aclarado su manejo terapéutico óptimo. A pesar de la controversia en cuanto al manejo quirúrgico de estas lesiones, el diagnóstico precoz, por aspiración esterotáxica, y la iniciación de una terapia antimicrobiana son esenciales para la buena evolución del paciente. Se presenta una paciente portadora de una poliglobulia primaria, con absceso cerebral recidivante por Nocardia asteroides de posible origen dentario.
