Objetivo: verificar a associação entre perfil bacteriológico e variáveis relacionadas às feridas neoplásicas dos pacientes sob cuidados paliativos. Método: Estudo transversal, com amostra composta por 36 pacientes internados nas enfermarias do Instituto Nacional do Câncer José Alencar Gomes da Silva – Unidade IV. A coleta de dados ocorreu entre os meses de outubro e dezembro de 2018, sendo utilizado um formulário próprio. Na análise dos dados utilizaram-se medidas descritivas e teste de Fischer/qui-quadrado ao nível de significância de 5% (α = 0,05). Resultados: A amostra foi composta por 36 indivíduos, sendo 19 (52,78%) do sexo feminino, com percentual predominante de formação de metástases (63,89%). Há forte associação entre as variáveis radioterapia paliativa e a presença de Pseudomonas sp. (p = 0,02), tipo de tumor primário e a presença de Citrobacter sp. e Acinetobacter sp. (p < 0,001) e localização da ferida neoplásica a presença de Citrobacter sp. (p = 0,03) e Acinetobacter sp. (p = 0,01). A não utilização de antibiótico não favorece o aumento do biofilme na ferida neoplásica. A quimioterapia paliativa não se associa a nenhum gênero bacteriano. As principais bactérias identificadas foram Pseudômonas sp., Morganella sp., Klebsiella sp. e Proteus sp.. Conclusão: o tratamento com radioterapia, o tipo de tumor primário e local da ferida tumoral apresentam associação significativa com o perfil bacteriológico das lesões e, portanto, recomenda-se a coleta de swab e análise de antibiograma das feridas neoplásicas para aprimorar a assistência de enfermagem.
RESUMO Objetivo: discutir os cuidados de Enfermagem relacionados às feridas neoplásicas nos pacientes em cuidados paliativos. Método: trata-se de um estudo bibliográfico, tipo revisão integrativa, com buscas nas bases de dados Cochrane, LILACS, MEDLINE, BVS e Portal CAPES, em publicações nos idiomas português, espanhol e inglês, com recorte temporal de 2012 a 2017, selecionou-se 7 artigos para a análise descritiva. Resultados: encontraram-se nos artigos selecionados as seguintes categorias de acordo com a sintomatologia abordada: Controle da dor; Controle do odor; Prevenção de hemorragias; Controle de exsudato e Aspectos psicossociais. Conclusão: norteiam-se, pelos cuidados de Enfermagem encontrados, as condutas do enfermeiro ao realizar os curativos das feridas neoplásicas. Destaca-se, no entanto, um importante aspecto que evidencia o comprometimento psicossocial que envolve o paciente com ferida neoplásica, considerando este, também, um cuidado de Enfermagem a ser realizado. Background The needs of patients with cancer must be met, especially in times of crisis. The advent of the pandemic triggered a series of strategic actions by the nursing team to preserve the health of patients and professionals—hence the importance of studies on nursing care actions provided to patients with cancer during the COVID-19 pandemic. It is known that these patients are susceptible to severe COVID-19. However, no previous review has summarized the findings of scientific studies on nursing for COVID-19 in patients with cancer. Objective This study aims to map the topics addressed in scientific studies on nursing for COVID-19 in patients with cancer. Methods A scoping review was conducted using the methodology described in the Joanna Briggs Institute Reviewers' Manual 2015. The research question was elaborated using the population, concept, and context framework: What topics have been studied in nursing publications about COVID-19 in adult patients with cancer? The searches were carried out in 8 databases between April and November 2021 without time restrictions. Results In total, 973 publications were identified using the search strategies in the databases, and 12 papers were retrieved by consulting the references. A total of 31 (3.2%) publications were included in the final analysis, generating 4 thematic categories on the subject: “restructuring the services: how oncology nursing was adapted during the pandemic,” “experiences of patients and performance of the nursing team during the COVID-19 pandemic,” “protocols and recommendations for dealing with the COVID-19 pandemic,” and “challenges and the role of oncology nurses facing the COVID-19 pandemic.” Conclusions Several strategies used by oncology nurses to face the COVID-19 pandemic in the international scenario were identified. Reports about the restructuring of services and the team's reactions to the pandemic predominated. However, there is a lack of reports regarding emotional support strategies for health care professionals. Another gap identified was the scarcity of clinical studies on the activities developed by oncology nurses. Therefore, there is a need for clinical research in the oncology area and emotional coping strategies to support oncology nurses.
Este estudo objetiva analisar o perfil dos pacientes admitidos em uma unidade de terapia intensiva oncológica. Trata-se de um estudo descritivo de abordagem quantitativa. A coleta de dados foi realizada no período de setembro a dezembro de 2016, através de sistema informatizado utilizado pela instituição. Foram admitidos 55 pacientes durante o período da coleta. A idade variou entre 18 e 80 anos, com média de 55 anos. Em sua maioria, são pacientes com neoplasias gastrointestinais (29,09%), provenientes das enfermarias do hospital (32,73%). No geral, 14,55% receberam cuidados paliativos e a mortalidade total no período foi de 34,55%. A carga de trabalho apresentada foi de 79,04%, traduzida em uma demanda de trabalho de 18 horas e 57 minutos. Houve predomínio do Karnofsky Performance Status 30% (52,73%) e de pacientes com Índice de Comorbidade de Charlson entre 2 e 5 pontos (76,36%). As unidades de terapia intensiva oncológica devem estar preparadas para atuar com pacientes sem possibilidades de cura. A mortalidade não é superior quando comparada a unidades de terapia intensiva de outras especialidades, desmistificando a admissão de pacientes oncológicos em unidades de terapia intensiva.
Este estudo tem como objetivo mapear as evidências científicas sobre intervenções de enfermagem relacionadas ao manejo de reações adversas em pacientes oncológicos adultos em uso de imunoterapia no ambiente ambulatorial. Trata-se de uma revisão de escopo, a partir da metodologia JBI. Utilizou-se como estratégia a população dos pacientes oncológicos adultos e idosos em uso de imunoterapia, no conceito das intervenções de enfermagem na administração e controle de reações adversas de imunoterápicos injetáveis, tendo como contexto o ambiente ambulatorial (PCC), em consonância com as diretrizes JBI para revisão de escopo. Foram selecionados 23 artigos, com publicações entre 2005 e 2020. Evidenciou-se três aspectos para discussão: Intervenções de enfermagem em relação a educação em saúde paciente/ família em uso de imunoterápicos, intervenções de enfermagem relacionadas a aplicação dos imunoterápicos no paciente e intervenções relacionadas as reações adversas após a aplicação dos imunoterápicos. É evidente o quanto faz-se necessário o exame físico realizado pelo enfermeiro e o conhecimento farmacológico sobre os imunoterápicos. Sugere-se que a sistematização da assistência de enfermagem seja presente nas instituições de saúde, e que haja elaboração de protocolos institucionais que facilitem a prática profissional de enfermagem em imunoterapia.
