Volunteers living in an area where schistosomiasis mansoni is endemic were subjected to ultrasound examination and classified into groups according to the levels of fibrosis diagnosed, namely, absence of indications of fibrosis (group 0), incipient fibrosis (group 1), and moderate/severe fibrosis (group 2). Peripheral blood mononuclear cells (PBMC) collected from the volunteers were stimulated with soluble antigens from adult schistosomes or from schistosome eggs, and the production of the cytokines gamma interferon, tumor necrosis factor alpha, transforming growth factor  (TGF-), interleukin-4 (IL-4), IL-10, and IL-13 was determined. Potential associations of the level of fibrosis with age, sex, intensity of infection, and cytokine production were investigated between the three groups. Univariate analysis identified associations of age (>50), gender (male), and absence of eggs/g of feces with moderate/severe fibrosis and an association of intensity of infection (>100 eggs) with incipient fibrosis. When cytokine production in PBMC cultures stimulated by soluble egg antigens was categorized as low or high, significant differences in the distribution of IL-13 levels were established between groups 0 and 2. No significant differences were detected between the groups in the cytokines produced by PBMC cultures stimulated with soluble antigens from adult schistosomes. When all variables were tested in multivariate analyses, only IL-13 was strongly associated with fibrosis (odds ratio ؍ 5.8; 95% confidence interval [CI] ؍ 1
Background Leishmania (Viannia) braziliensis has been associated with a broad range of clinical manifestations ranging from a simple cutaneous ulcer to destructive mucosal lesions. Factors leading to this diversity of clinical presentations are not clear, but parasite factors have lately been recognized as important in determining disease progression. Given the fact that the activity of ecto-nucleotidases correlates with parasitism and the development of infection, we evaluated the activity of these enzymes in promastigotes from 23 L. braziliensis isolates as a possible parasite-related factor that could influence the clinical outcome of the disease.Methodology/Principal FindingsOur results show that the isolates differ in their ability to hydrolyze adenine nucleotides. Furthermore, we observed a positive correlation between the time for peak of lesion development in C57BL/6J mice and enzymatic activity and clinical manifestation of the isolate. In addition, we found that L. (V.) braziliensis isolates obtained from mucosal lesions hydrolyze higher amounts of adenine nucleotides than isolates obtained from skin lesions. One isolate with high (PPS6m) and another with low (SSF) ecto-nucleotidase activity were chosen for further studies. Mice inoculated with PPS6m show delayed lesion development and present larger parasite loads than animals inoculated with the SSF isolate. In addition, PPS6m modulates the host immune response by inhibiting dendritic cell activation and NO production by activated J774 macrophages. Finally, we observed that the amastigote forms from PPS6m and SSF isolates present low enzymatic activity that does not interfere with NO production and parasite survival in macrophages.Conclusions/SignificanceOur data suggest that ecto-nucleotidases present on the promastigote forms of the parasite may interfere with the establishment of the immune response with consequent impaired ability to control parasite dissemination and this may be an important factor in determining the clinical outcome of leishmaniasis.

A esquistossomose mansoni é causada pelo helminto S.mansoni e configura-se como um importante problema de saúde pública. Entre as formas clínicas, ressalta-se a hepatoesplênica, na qual há formação de fibrose periportal, podendo culminar em hipertensão portal, insuficiência hepática e óbito. Diante da possibilidade de quadros graves, justifica-se a relevância de estudos epidemiológicos em áreas endêmicas, posto que essas pesquisas viabilizam o direcionamento de intervenções. Assim, o objetivo do presente trabalho consiste em caracterizar aspectos espaciais, sociodemográficos, clínicos, e temporais da esquistossomose em Minas Gerais, entre os anos de 2011 e 2020. Trata-se de um estudo ecológico com dados secundários de domínio público, disponíveis no Portal de Vigilância da Secretaria de Estado de Saúde de Minas Gerais. Totalizaramse 37.535 casos, dos quais 159 evoluíram para óbito (letalidade de 0,4%). Houve destaque para as macrorregiões Vale do Aço (n=10.438; 27,8), Nordeste (n=8.327; 22,2%) e Centro (n=5.928; 15,7%); e para os municípios de Belo Horizonte (n=3.148; 8,3%),
A sífilis congênita corresponde à infecção do feto pelo T.pallidum, principalmente por via transplacentária, podendo culminar em perda fetal, natimorto, morte neonatal e prematuridade. O objetivo do presente trabalho consiste em descrever as hospitalizações por sífilis congênita no Brasil, entre os anos de 2008 e 2017. Trata-se de um estudo ecológico conduzido a partir de dados secundários de domínio público, vinculados ao Sistema de Informações Hospitalares do Sistema Único de Saúde (SIH/SUS). Para caracterização, incluíram-se as variáveis: região do Brasil, unidade da federação, sexo, raça/cor, caráter de atendimento, regime de internação, óbitos, ano e gastos totais. Para cálculo dos coeficientes de internação, o número de nascidos vivos foi obtido do Sistema de Informação sobre Nascidos Vivos (SINASC). Totalizaram-se 78.724 internações por sífilis congênita em todo o período estudado, das quais 321 evoluíram para óbito (letalidade hospitalar de 0,4%). No que concerne às regiões brasileiras, houve ênfase para Sudeste (n=30.339; 38,5%), Nordeste (n=28.093; 35,6%) e Norte (n=9.071; 11,5%). Com relação aos coeficientes de internação, foram registrados 2,5 internados para cada 1.000 nascidos vivos considerando todo o território nacional, sendo importante realçar o Nordeste (3,8 internados para cada 1.000 nascidos vivos). Paralelamente, as unidades da federação de destaque foram Rio de Janeiro (n=13.015; 16,5%), São Paulo (n=10.810; 13,7%) e Pernambuco (n=8.093; 10,3%). O perfil geográfico de distribuição de óbitos foi semelhante ao de hospitalizações, com ênfase para: regiões Nordeste (n=149/321; 46,4%) e Sudeste (n=97/321; 30,2%); e estados de Pernambuco (n=69/321; 21,4%) e Rio de Janeiro (n=38/321; 11,8%). A maior letalidade hospitalar foi observada no Nordeste (0,5%). Salienta-se ainda sexo feminino (n=41.324; 52,4%) e raça/cor parda (n=27.241; 34,6%). A maior parcela dos atendimentos ocorreu em caráter de urgência (n=74.785; 95%) e no regime público (n=31.211; 39,65%). Acumularam-se R$ 58.085.276,75 em gastos. Do ponto de vista temporal, ocorreu aumento no número de internações hospitalares por sífilis congênita no Brasil de 3.570 (4,5%) no ano de 2008 para 15.446 (19,6%) hospitalizações em 2017. Tal progressão refletiu diretamente nos coeficientes de internação, cujo menor valor foi 1,22 acometidos para cada 1.000 nascidos vivos em 2008 para 5,29 acometidos por 1.000 nascidos vivos em 2017. Percebe-se que a sífilis congênita é um problema de impacto para a saúde pública brasileira, sendo necessária a adoção de profilaxias.
