Introducción: las enfermedades prevenibles por vacunas y las inmunizaciones representan una prioridad para el Ministerio de Salud Pública de República Dominicana; de esto depende, en gran medida, la salud de la población. Por ello, es de gran interés determinar los conocimientos, actitudes y prácticas sobre vacunación e inmunización del personal de puestos de vacunas. Método: es un estudio descriptivo-transversal sobre conocimientos, actitudes y prácticas de vacunación e inmunización en personal de puestos de vacunas de Santo Domingo y el Distrito Nacional en 2010. Se elaboró un cuestionario tomando como base las variables e indicadores de este estudio y se aplicó a 257 miembros de puestos de vacuna, de un universo de 415 integrantes. Resultados: más del 90 % confirmó que las vacunas evitan enfermedades, que en una sesión se pueden aplicar diferentes vacunas, que deben conservarse entre +2 y +8 °C. Más del 90 % tiene actitudes favorables en cuanto a la importancia de vacunar y su registro, orientación a las madres y el lavado de manos antes de vacunar. La actitud menos favorable fue en relación con la afirmación de que dar masajes es una acción a realizar después de aplicar cualquier vacuna. El 90 % utiliza caja de bioseguridad, nevera exclusiva de vacunas y con termómetros funcionales. El 33 % deposita jeringuillas en zafacón y 27% tapa nuevamente jeringuilla. Conclusiones: el personal tiene conocimientos adecuados sobre vacunación e inmunización, existe falta de correspondencia entre porcentajes de conocimientos adecuados y actitudes favorables con las prácticas de este personal frente a las acciones de inmunización.
Introducción: la vigilancia centinela de Infecciones Respiratorias Agudas Graves (IRAG) inició en República Dominicana en febrero del año 2008, en dos hospitales: el Hospital Infantil Dr. Robert Reid Cabral (HIRRC) en Santo Domingo y el Hospital José María Cabral y Báez, en Santiago. Este estudio tiene como objetivo general describir el comportamiento de las infecciones respiratorias agudas graves (IRAG) captadas en el Hospital Infantil Dr. Robert Reid Cabral durante los años 2015-2019. Método: se realizó un estudio observacional, descriptivo, de corte transversal, con recolección retrospectiva de datos, de los casos de IRAG reportados en el puesto centinela del Hospital Infantil de Referencia Nacional Dr. Robert Reid Cabral, Área V de salud, Distrito Nacional, desde el año 2015 hasta el año 2019. Se hace una descripción de las características de los casos ingresados a la base de datos del sistema, los virus respiratorios identificados, comportamiento en el tiempo, persona y lugar. Resultados: la captación de infecciones respiratorias agudas graves (IRAG) a través del puesto centinela las IRAG, representaron 14.37, 10.13, 4.10, 3.70 y 3.42 % de las IRA, respectivamente, en los últimos cinco años. En las semanas epidemiológicas, en el año 2015 hubo una mayor cantidad de casos entre las semanas 9-16 y 40-48, con un pico alto en la semana 47, en el año 2016 estuvo concentrado entre las semanas 24-42, en el año 2017, en las semanas 33-38, en el año 2018 se presentaron con mayor frecuencia entre las semanas 10-15 y con altas y bajas entre la semana 25-49, y ya para el año 2019 se reportaron casos de manera irregular con altas y baja durante la semana 6-41. La detección de la circulación del Virus Sincitial Respiratorio (VSR) se identificó puntualmente con mayor porcentaje en los años 2015, 2016, 2017 y el año 2019 en comparación con el año 2018 donde el agente predominante fue el virus de la influenza A H1N1. El mayor pico de mortalidad por IRAG estuvo presente en el año 2015 con 18 muertes. Conclusión: la captación de infecciones respiratorias agudas graves (IRAG) a través del puesto centinela disminuyó en los años 2017 (501 casos), 2018 (465 casos) y 2019 (381 casos), debido, principalmente, a una modificación de la definición de casos establecida por la OPS/OMS y remitida por la Dirección General de Epidemiología, representando solo un 4.10, 3.70 y 3.42 %, respectivamente, de las IRA en esos últimos tres años de estudio; sin embargo, en los 2 primeros años de estudio, 2015 (1706 casos) y 2016 (1350 casos), cuando no se habían disminuido los criterios de definición de caso, las IRAG representaron 14.37 y 10.13 % de las IRA, respectivamente.
Introducción: la difteria es una enfermedad infecciosa causada, principalmente, por cepas toxigénicas del Corynebacterium diphtheriae, las infecciones locales pueden afectar tanto el tracto respiratorio como el sistema tegumentario.1, 2 En República Dominicana, el último brote de difteria ocurrido data del año 2004. Posterior al mismo, la incidencia de esta enfermedad ha experimentado una disminución considerable, registrándose un promedio de 2.6 casos por año (rango= 1-7) en los últimos cinco años. El objetivo de esta investigación es describir 21 casos probables de difteria notificados durante el período enero-mayo 2021. Método: se realizó un reporte descriptivo a partir del análisis retrospectivo del formulario único de notificación de caso y los expedientes de 21 pacientes con diagnóstico probable de difteria, que fueron admitidos al Hospital Infantil Dr. Robert Reid Cabral, durante el período enero-mayo de 2021. Resultados: el grupo de edades más afectado fue el de los niños entre 1-4 años con 11 casos (52 %). Se presentó historia de estatus de inmunización incompleto en 19 casos (90.5 %). Se confirmó el diagnóstico de difteria en nueve casos (42.9 %). La antitoxina diftérica (DAT) fue administrada en 17 casos (81 %), 11 de estos pacientes resultaron fallecidos y 6 sobrevivieron, mientras que, de los 4 pacientes que no recibieron la (DAT) (19 %) 1 falleció (p= 0.272). La semana epidemiológica con mayor registro de inicio de casos fue la semana quince con 6 casos (28 %). El tipo de complicación más frecuente fue la insuficiencia respiratoria, presentada en un 50 % de los casos. Conclusiones: de los 21 casos, 12 fallecieron, lo cual representa una tasa de letalidad de un 58 %, cifras que superan la tasa de letalidad de un 32 % registrada en el brote del año 2004. Se presentó historia de estatus de inmunización incompleto en 19 casos (90.5 %), lo cual sugiere que uno de los detonantes del brote sea la caída de la inmunidad inducida por las vacunas en el tiempo, y la falta de dosis de refuerzos. Considerando las limitaciones de producción y distribución de la toxina que aún prevalecen y afectan mayormente a países en vías de desarrollo, el hecho de que un 81 % de la totalidad de los pacientes recibió la DAT es un dato sobresaliente en comparación con otros países de las Américas.

Introducción: Tras 18 casos de poliomielitis descubiertos al final del 2000, en Constanza, el Ministerio de Salud Pública (MSP) y la Organización Panamericana de la Salud (OPS), comparten la responsabilidad de revisar, a los 60 días, todos los casos de Parálisis Flácidas Agudas (PFA). Para examinarlos se nombra un comité de profesores (Tolentino, Mendoza, Santoni). Las entidades comprometidas habían publicado las características clínico-epidemiológicas del brote, mientras que J.Santoni y C.Santoni Williams publican su electromiografía. Para este artículo se revisa la literatura de etiopatogenias en la PFA y polio. Métodos: Se citan 336 casos acompañados por encargados comunitarios, 188 varones, 148 hembras, edad promedio: 6.2 años. Se revisa: evolución, imágenes, analítica, cultivo viral en heces. Así mismo, diagnóstico y clasificación tras examen neurológico. Resultados: Casos de poliomielitis (18), de 1 a 13 años: todos vacunados irregularmente. Citamos patología de membrana CD155 y del cañón neuronal. Describimos los casos de PFA en orden descendente; síndromes de Guillain-Barré (84), 10 de ellos Miller-Fisher; evolución moderada, 2 fallecidos. Se atribuye a mímica proteica de oligosacáridos. Casos de FPA que involucran a las citokinas; mielitis y encefalomielitis diseminada agudas (58); encefalitis, meningitis, cerebelitis (49); polineuritis/mononeuritis (23); miopatías relacionadas a Estreptococias (25) con sus 6 Coreas por mímica proteica anti-lysogangliosido GM1; procesos entero virales (16), vasculitis con ACV escasos (11), tumores (10). Finalmente, encontramos 15 con exámenes asimétricos sospechosos de motoneurona lesionada, pero todos con cultivos fecales de poliovirus negativos; y 24 exámenes neurológicos normales, pero sin causa. Aceptamos 3 inasistencias. Conclusión: Demostramos la ausencia de nuevos casos de polio y actualizamos la clasificación de las PFA. Revisando sus etiopatogenias las entendemos mejor.
