RESUMO O processo de regionalização proposto pelo Ministério da Saúde define região de saúde como a base territorial de planejamento da atenção à saúde da saúde e que deve levar em consideração as características demográficas, sócio-econômicas, geográficas, sanitárias, epidemiológicas. Essa concepção de região é bem mais flexível do que as tradicionalmente adotadas nos programas de regionalização. Mas os documentos já elaborados indicam uma tendência a tratar as regiões como unidades de intervenção e ação do Estado, deixando pouco espaço para a participação da sociedade civil. Requer-se, pois, que a delimitação dessas regiões leve em consideração as redes de articulação regional já existentes e que se aproveitem os elementos capazes de contribuir para o fortalecimento da identidade regional. Identidade esta que deve provir das experiências pessoais dos indivíduos no território de limitado pela região e que representem sua consciência regional.
Objetivo: Conhecer a aceitação de pacientes de ensaio clínico quanto a coleta de exames domiciliares, testes remotos e teleconsultas, compreendendo as suas preferências e expectativas. Método: Estudo qualitativo, exploratório, descritivo, com entrevistas em profundidade envolvendo profissionais de saúde (especialistas) e pacientes, com e sem experiência em ensaios clínicos, com posterior validação dos constructos por meio de workshop participativo. Resultados: Para os pacientes de ensaio clínico, a aceitação mostrou-se relativa. Diversos fatores influenciam suas preferências e expectativas, sendo as principais: a) O grau de confiabilidade nos processos e a acurácia dos resultados de exames domiciliares. b) Os testes remotos, apesar de bem aceitos, dependem da capacidade de entendimento no correto manuseio da tecnologia requerida. c) A teleconsulta, já bem utilizada e aceita, não substitui totalmente o encontro presencial com o médico em fases críticas. A preferência é pelo uso equilibrado do “presencial x remoto”. Conclusão: A aceitação é influenciada por diversas variáveis. Devem ser definidas estratégias adequadas para cada caso, para garantir resultados seguros e confiáveis, com equilíbrio entre o presencial e remoto, através da participação ativa dos pacientes nas tomadas de decisão. O Consórcio Brasileiro de Acreditação, instituição não governamental sem fins lucrativos, constituído em 1998 na cidade do Rio de Janeiro, é o representante, no Brasil, da metodologia de acreditação internacional da The Joint Commission. Fundada em 1951, é a maior, mais antiga e experiente organização acreditadora do mundo, que avalia mais de 22.000 instituições de saúde nos Estados Unidos e mais de 730 no restante do mundo, por meio de sua divisão internacional, a Joint Commission International. Ao atuar em associação com a Joint Commission International, o Consórcio Brasileiro de Acreditação possibilitou, no Brasil, a implantação de uma metodologia já consistente e reconhecida mundialmente. Sua metodologia utiliza um conjunto de padrões de excelência, em forma de manuais, que sofrem revisões e atualizações periódicas. A qualidade, as práticas baseadas em evidência e a cultura de segurança do paciente nas instituições de saúde são aspectos fundamentais e embasam seus padrões. O método do rastreador (tracer method) é amplamente utilizado nas avaliações da Joint Commission International / Consórcio Brasileiro de Acreditação e representa cerca de 60 a 70% das atividades realizadas durante o processo avaliativo. O tracer permite identificar se existem descontinuidades ou quebras nos sistemas, sendo muito mais eficiente do que o método tradicional, por ser mais processual do que departamental.
