Objectives We sought to measure the strength of association between two indices of obesity, waist hip ratio (WHR) and body mass index (BMI), with left ventricular (LV) dysfunction and mortality in a community cohort. Background The distribution of body fat is thought to affect cardiovascular disease risk. The association of BMI (an index of overall obesity) and WHR (an index of central obesity) with LV systolic and diastolic dysfunction in a population-based cohort is unknown. Methods Results Anthropomorphic measurements and echocardiographic LV function were measured in a cross-sectional population-based sample of 2042 men and women >45 years old in the Olmsted County Heart Function Study. Five year prospective mortality was measured. Results Increased WHR had a stronger association than BMI with: 1) lower LV EF (r= -0.24, p<0.0001 versus r= -0.04, p=0.09), and 2) LV diastolic dysfunction (r = 0.18, p<0.0001 versus r = 0.05, p = 0.02). After adjustment for standard cardiovascular risk factors WHR continued to be significantly associated with diastolic dysfunction, but not with systolic dysfunction. WHR, but not BMI, was strongly predictive of all cause mortality independent of age and sex (HR 23.6, CI 4.0, 139.8; p = 0.0005). This relationship was attenuated on adjustment for diastolic dysfunction. Conclusions WHR is a stronger correlate of LV dysfunction and mortality than BMI. These cross-sectional data suggest that the increased risk of mortality from central obesity is mediated at least in part by LV dysfunction, especially diastolic dysfunction.

Background Myocarditis following coronavirus disease 2019 (COVID-19) mRNA vaccines (Pfizer-BioNTech and Moderna) has been increasingly reported. Incidence rates in the general population are lacking, with pericarditis rather than myocarditis diagnostic codes being used to estimate background rates. This comparison is critical for balancing the risk of vaccination with the risk of no vaccination. Methods A retrospective case series was performed using the Mayo Clinic COVID-19 Vaccine Registry. We measured the incidence rate ratio (IRR) for myocarditis temporally related to COVID-19 mRNA vaccination compared with myocarditis in a comparable population from 2016 through 2020. Clinical characteristics and outcomes of the affected patients were collected. A total of 21 individuals were identified, but ultimately 7 patients met the inclusion criteria for vaccine-associated myocarditis. Results The overall IRR of COVID-19–related myocarditis was 4.18 (95% confidence interval [CI], 1.63–8.98), which was entirely attributable to an increased IRR among adult males (IRR, 6.69; 95% CI, 2.35–15.52) compared with females (IRR 1.41; 95% CI, .03–8.45). All cases occurred within 2 weeks of a dose of the COVID-19 mRNA vaccine, with the majority occurring within 3 days (range, 1–13) following the second dose (6 of 7 patients, 86%). Overall, cases were mild, and all patients survived. Conclusions Myocarditis is a rare adverse event associated with COVID-19 mRNA vaccines. It occurs in adult males with significantly higher incidence than in the background population. Recurrence of myocarditis after a subsequent mRNA vaccine dose is not known at this time.

Purpose: To estimate the ability of lower dose levels and iterative reconstruction (IR) to display hepatic metastases that can be detected by radiologists. Materials and Methods: Projection data from 83 contrast agent–enhanced CT examinations were collected. Metastases were defined by histopathologic analysis or progression and regression. Lower radiation dose configurations were reconstructed at five dose levels with filtered back projection (FBP) and IR (automatic exposure control settings: 80, 100, 120, 160, and 200 quality reference mAs [QRM]). Three abdominal radiologists circumscribed metastases, indicating confidence (confidence range, 0–100) and image quality. Noninferiority was assessed by using jackknife alternative free-response receiver operating characteristic (JAFROC) analysis (noninferiority limit, −0.10) and reader agreement rules, which required identification of metastases identified at routine dose, and no nonlesion localizations in patients negative for metastases, in 71 or more patient CT examinations (of 83), for each configuration. Results: There were 123 hepatic metastases (mean size, 1.4 cm; median volume CT dose index and size-specific dose estimate, 11.0 and 13.4 mGy, respectively). By using JAFROC figure of merit, 100 QRM FBP did not meet noninferiority criteria and had estimated performance difference from routine dose of −0.08 (95% confidence interval: −0.11, −0.04). Preset reader agreement rules were not met for 100 QRM IR or 80 QRM IR, but were met for doses 120 QRM or higher (ie, size-specific dose estimate ≥ 8.0 mGy). IR improved image quality (P < .05) but not reader performance. Other than 160 QRM IR, lower dose levels were associated with reduced confidence in metastasis detection (P < .001). Conclusion: For detection of hepatic metastases by using contrast-enhanced CT, dose levels that corresponded to 120 quality reference mAs (size-specific dose estimate, 8.0 mGy) and higher performed similarly to 200 quality reference mAs with filtered back projection.

Background Mental health after spontaneous coronary artery dissection ( SCAD ), a cause of myocardial infarction in young women, remains largely unexplored. We assessed the prevalence and severity of psychiatric symptoms after SCAD . Methods and Results Individuals with confirmed SCAD who consented to the Mayo Clinic “Virtual” Multicenter SCAD Registry were sent the Posttraumatic Stress Disorder Diagnostic Scale for Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition , Patient Health Questionnaire‐9, Generalized Anxiety Disorder‐7, Anxiety Sensitivity Index, Connor‐Davidson Resilience Scale, 36‐Item Short‐Form Health Survey, and an SCAD ‐specific questionnaire. Among 782 patients contacted, 512 surveys were returned. Most respondents were women (97.5%), with median age at time of SCAD and survey completion of 47 and 52 years, respectively. Eighty‐two percent had at least one trauma, with mild or more posttraumatic stress disorder symptoms in 28%. Symptoms of anxiety and depression were observed in 41% and 32%, respectively. On multivariable analysis, those of younger age at first SCAD and low resiliency scored higher on measures of trauma, anxiety, and depression. Those with higher anxiety sensitivity had more severe anxiety and posttraumatic stress disorder symptoms. Emotional and social quality of life was higher in those with high resiliency scores. Time from SCAD event to survey completion was associated with lower Generalized Anxiety Disorder‐7 score severity. Conclusions Survivors of SCAD have significant rates of posttraumatic stress disorder, depression, and anxiety, which are associated with lower quality of life specifically among those with lower resiliency. Given the prevalence and potential impact, screening and treatment for the psychological distress is advised. Behavioral interventions targeted toward resiliency training may be beneficial for this patient population.
