Better understanding of the dose-toxicity relationship is critical for safe dose escalation to improve local control in late-stage cervical cancer radiotherapy. In this study, we introduced a convolutional neural network (CNN) model to analyze rectum dose distribution and predict rectum toxicity. Forty-two cervical cancer patients treated with combined external beam radiotherapy (EBRT) and brachytherapy (BT) were retrospectively collected, including twelve toxicity patients and thirty non-toxicity patients. We adopted a transfer learning strategy to overcome the limited patient data issue. A 16-layers CNN developed by the visual geometry group (VGG-16) of the University of Oxford was pre-trained on a large-scale natural image database, ImageNet, and fine-tuned with patient rectum surface dose maps (RSDMs), which were accumulated EBRT  +  BT doses on the unfolded rectum surface. We used the adaptive synthetic sampling approach and the data augmentation method to address the two challenges, data imbalance and data scarcity. The gradient-weighted class activation maps (Grad-CAM) were also generated to highlight the discriminative regions on the RSDM along with the prediction model. We compare different CNN coefficients fine-tuning strategies, and compare the predictive performance using the traditional dose volume parameters, e.g. D , and the texture features extracted from the RSDM. Satisfactory prediction performance was achieved with the proposed scheme, and we found that the mean Grad-CAM over the toxicity patient group has geometric consistence of distribution with the statistical analysis result, which indicates possible rectum toxicity location. The evaluation results have demonstrated the feasibility of building a CNN-based rectum dose-toxicity prediction model with transfer learning for cervical cancer radiotherapy.

The dual-energy CT-based (DECT) approach holds promise in reducing the overall uncertainty in proton stopping-power-ratio (SPR) estimation as compared to the conventional stoichiometric calibration approach. The objective of this study was to analyze the factors contributing to uncertainty in SPR estimation using the DECT-based approach and to derive a comprehensive estimate of the range uncertainty associated with SPR estimation in treatment planning. Two state-of-the-art DECT-based methods, the Hünemohr-Saito method (2014, 2012) and the Bourque method (2014), were selected and implemented on a Siemens SOMATOM Force DECT scanner. The uncertainties were first divided into five independent categories. The uncertainty associated with each category was estimated for lung, soft and bone tissues separately. A single composite uncertainty estimate was eventually determined for three tumor sites (lung, prostate and head-and-neck) by weighting the relative proportion of each tissue group for that specific site. The uncertainties associated with the two selected DECT methods were found to be similar, therefore the following results applied to both methods. The overall uncertainty (1σ) in SPR estimation with the DECT-based approach was estimated to be 3.8%, 1.2% and 2.0% for lung, soft and bone tissues, respectively. The dominant factor contributing to uncertainty in the DECT approach was the imaging uncertainties, followed by the DECT modeling uncertainties. Our study showed that the DECT approach can reduce the overall range uncertainty to approximately 2.2% (2σ) in clinical scenarios, in contrast to the previously reported 1%.

Cone-beam CT (CBCT) has become the standard image guidance tool for patient setup in image-guided radiation therapy. However, due to its large illumination field, scattered photons severely degrade its image quality. While kernel-based scatter correction methods have been used routinely in the clinic, it is still desirable to develop Monte Carlo (MC) simulation-based methods due to their accuracy. However, the high computational burden of the MC method has prevented routine clinical application. This paper reports our recent development of a practical method of MC-based scatter estimation and removal for CBCT. In contrast with conventional MC approaches that estimate scatter signals using a scatter-contaminated CBCT image, our method used a planning CT image for MC simulation, which has the advantages of accurate image intensity and absence of image truncation. In our method, the planning CT was first rigidly registered with the CBCT. Scatter signals were then estimated via MC simulation. After scatter signals were removed from the raw CBCT projections, a corrected CBCT image was reconstructed. The entire workflow was implemented on a GPU platform for high computational efficiency. Strategies such as projection denoising, CT image downsampling, and interpolation along the angular direction were employed to further enhance the calculation speed. We studied the impact of key parameters in the workflow on the resulting accuracy and efficiency, based on which the optimal parameter values were determined. Our method was evaluated in numerical simulation, phantom, and real patient cases. In the simulation cases, our method reduced mean HU errors from 44 HU to 3 HU and from 78 HU to 9 HU in the full-fan and the half-fan cases, respectively. In both the phantom and the patient cases, image artifacts caused by scatter, such as ring artifacts around the bowtie area, were reduced. With all the techniques employed, we achieved computation time of less than 30 sec including the time for both the scatter estimation and CBCT reconstruction steps. The efficacy of our method and its high computational efficiency make our method attractive for clinical use.

Computed tomography (CT) to cone-beam computed tomography (CBCT) deformable image registration (DIR) is a crucial step in adaptive radiation therapy. Current intensity-based registration algorithms, such as demons, may fail in the context of CT-CBCT DIR because of inconsistent intensities between the two modalities. In this paper, we propose a variant of demons, called Deformation with Intensity Simultaneously Corrected (DISC), to deal with CT-CBCT DIR. DISC distinguishes itself from the original demons algorithm by performing an adaptive intensity correction step on the CBCT image at every iteration step of the demons registration. Specifically, the intensity correction of a voxel in CBCT is achieved by matching the first and the second moments of the voxel intensities inside a patch around the voxel with those on the CT image. It is expected that such a strategy can remove artifacts in the CBCT image, as well as ensuring the intensity consistency between the two modalities. DISC is implemented on computer graphics processing units (GPUs) in compute unified device architecture (CUDA) programming environment. The performance of DISC is evaluated on a simulated patient case and six clinical head-and-neck cancer patient data. It is found that DISC is robust against the CBCT artifacts and intensity inconsistency and significantly improves the registration accuracy when compared with the original demons.

Purpose: The purpose of this study is to investigate the effect of different magnetic resonance (MR) sequences on the accuracy of deep learning-based synthetic computed tomography (sCT) generation in the complex head and neck region. Methods: Four sequences of MR images (T1, T2, T1C, and T1DixonC-water) were collected from 45 patients with nasopharyngeal carcinoma. Seven conditional generative adversarial network (cGAN) models were trained with different sequences (single channel) and different combinations (multi-channel) as inputs. To further verify the cGAN performance, we also used a U-net network as a comparison. Mean absolute error, structural similarity index, peak signal-to-noise ratio, dice similarity coefficient, and dose distribution were evaluated between the actual CTs and sCTs generated from different models. Results: The results show that the cGAN model with multi-channel (i.e., T1 + T2 + T1C + T1DixonC-water) as input to predict sCT achieves higher accuracy than any single MR sequence model. The T1-weighted MR model achieves better results than T2, T1C, and T1DixonC-water models. The comparison between cGAN and U-net shows that the sCTs predicted by cGAN retains additional image details are less blurred and more similar to the actual CT. Conclusions: Conditional generative adversarial network with multiple MR sequences as model input shows the best accuracy. The T1-weighted MR images provide sufficient image information and are suitable for sCT prediction in clinical scenarios with limited acquisition sequences or limited acquisition time.

Deformable image registration (DIR) is a critical technic in adaptive radiotherapy (ART) for propagating contours between planning computerized tomography (CT) images and treatment CT/cone-beam CT (CBCT) images to account for organ deformation for treatment re-planning. To validate the ability and accuracy of DIR algorithms in organ at risk (OAR) contour mapping, ten intensity-based DIR strategies, which were classified into four categories—optical flow-based, demons-based, level-set-based and spline-based—were tested on planning CT and fractional CBCT images acquired from twenty-one head & neck (H&N) cancer patients who underwent 6~7-week intensity-modulated radiation therapy (IMRT). Three similarity metrics, i.e., the Dice similarity coefficient (DSC), the percentage error (PE) and the Hausdorff distance (HD), were employed to measure the agreement between the propagated contours and the physician-delineated ground truths of four OARs, including the vertebra (VTB), the vertebral foramen (VF), the parotid gland (PG) and the submandibular gland (SMG). It was found that the evaluated DIRs in this work did not necessarily outperform rigid registration. DIR performed better for bony structures than soft-tissue organs, and the DIR performance tended to vary for different ROIs with different degrees of deformation as the treatment proceeded. Generally, the optical flow-based DIR performed best, while the demons-based DIR usually ranked last except for a modified demons-based DISC used for CT-CBCT DIR. These experimental results suggest that the choice of a specific DIR algorithm depends on the image modality, anatomic site, magnitude of deformation and application. Therefore, careful examinations and modifications are required before accepting the auto-propagated contours, especially for automatic re-planning ART systems.
