The acute development and the accumulation of chyle into the peritoneal space is a rare condition and one needs to be able to distinguish it from chylous ascites, which is most commonly chronic and associated with related diseases. It is frequently idiopathic and additionally, clinical and imaging findings are nonspecific. In almost all cases, the diagnosis is made intraoperatively during laparoscopy or laparotomy, most diagnostic procedures being indicated by acute complaints pertaining to the abdomen. A 23-year-old man was admitted to the Emergency Department with abdominal pain, with right iliac fossa tenderness and peritonism. Laparoscopy was performed due to acute abdomen findings. Further exploration revealed chylous effusion with milky-like fluid. The peritoneal lavage and the insertion of drains were subsequent to the careful inspection of the cavity. The biochemical analysis of the peritoneal fluid was an important aid which provided the diagnosis of chylous peritonitis due to acute pancreatitis.
Background. This study evaluated the difference between boys and girls in terms of nutritional status, lifestyle, and dietary habits during school life. Materials and Methods. A descriptive and observational study was conducted in 2016, in which 251 children, aged 7-17, from 3 elementary schools and a high school in Bucharest, Romania, were evaluated. A questionnaire was used to assess food behavior, eating, and lifestyle habits. Results. Boys had a significantly higher waist circumference (71.18±9) than girls (67.46±9.91) (p=0.004). Thus 27% of boys were overweight or obese compared with only 22% of the girls. Differences were also seen between the two groups in terms of main meals and snacks and following a rhythm of meals: a statistically significant percentage of girls (36.3%) skip breakfast, while most boys (63.8%) take a food package to school. A total of 23.8% of the boys and 24% of the girls state that they eat while sitting in front of the computer or TV. Conclusions. We found that boys are more overweight or obese than girls. Obesity in the pediatric population of Romania could be explained by the country's emergence from communism 25 years ago, pattern typical of all Eastern European countries and which currently involve an overexposure of people to fast food, fizzy drinks and sweets, as well as to a high consumption of salt and food additives. Unbalanced and highly caloric food had been preferable to healthy food in the last period. Leisure time is rather spent in front of the TV, tablet, detrimental to rational physical exercise, recreational sports or hiking. The family environment is very important and all our actions should be focused on continuous education about the risks of unhealthy food and a sedentary lifestyle.
Esophageal cancer represents a neoplasm that is thought to have both an increased incidence and prevalence in the following years. Although much progress has been made in the detection and the early treatment of esophageal cancer, the prognosis is still limited, and mortality remains very high. The most common histological types of esophageal cancer are squamous cell carcinoma, and, respectively, adenocarcinoma. Lately, there has been an accelerated increase in the incidence of adenocarcinoma, in the context of increased prevalence of gastro-esophageal reflux disease and obesity, but also of the current alimentary diet, especially in developed countries. The esophagus has its anatomical features. Moreover, it is located topographicallyin a complex cervico-thoraco-abdominal area, unique for a viscus. From the clinical point of view, the onset of symptoms of esophageal cancer is insidious. For these reasons, special attention should be pain in the early detection and differentiation of this neoplasm from other pathologies. These pathologies are very varied and may comprise other esophageal diseases, neighborhood pathologies such as cervical, thoracic, abdominal, systemic pathologies (immunologic, infectious) or other pathologies such as the oro-maxillo-facial, oculo-orbital, vascular, muscular, and cutaneous ones. New non-surgical esthetics is the most dynamic field in contemporary medicine. At the same time, it is greatly influenced by our contemporary society. This paper reviews modern methods used in non-surgical esthetics, especially in Romania. From injectology and non-surgical face lifting to non-surgical body contouring, we analyzed all procedures performed by Romanian physicians, and we show the advantages and disadvantages of the advanced esthetic procedures. Injectology typically implies hyaluronic acid, botox and mesotherapy (for wrinkles and rejuvenation). Laser and radiofrequency treatment are used for wrinkles as well, with fewer unwanted adverse reactions and results exceeding those of injectables. Non-surgical lifting has gained more ground because it requires little recovery time and the results are very positive. Elective treatment for facial discoloration is the laser approach. For non-surgical body contouring, cryolipolisis, vacuum, radio frequency, and infrared systems have all revolutionized this part of esthetics, but each has limitations, and only after establishing the correct course of action, might we think of achieving favorable results and thus raising to the expectations of patients.
